Care Coordinator - Home Care

Job Description

The Care Coordinator plays a crucial role in driving care consultations and sales initiatives, as well as fostering strategic partnerships within the community for our non-medical Home Care service organization. We are seeking a dynamic individual, ideally with local connections to spearhead our outreach efforts.

Care Coordinator Responsibilities:

· Conduct care consultations with potential clients to understand their needs and develop personalized care plans in collaboration with their families.

· Provide support and guidance to current clients through quality assurance visits, ensuring the delivery of exceptional care, and offering care coordination services as necessary.

· Cultivate partnerships with referral sources through educational initiatives, presentations, and events, serving as a knowledgeable resource for clients and their support systems.

· Drive sales and marketing efforts to expand business, leveraging existing relationships and establishing new connections within the community.

· Engage with caregivers to address their individual needs, reinforcing their importance as valued team members.

Care Coordinator Requirements:

· Demonstrated experience in building and nurturing positive relationships to drive business growth.

· Previous case management experience is strongly desired.

· Healthcare background or familiarity is preferred.

Note: Our Administration Office is in Fort Myers, FL. We also have a Satellite Office located in Naples, FL. This position would cover the Collier County area.

Salary: $60,000.00 - $70,000.00 per year base salary + bonus opportunities
