We provide In-Home care for seniors and older adults who require a helping hand, a supportive companion, and day-to-day assistance in their home. As an ECC you will have the opportunity to support our caregivers in delivering care that enhances the quality of life of our clients and their families.
JOB SUMMARY:
The External Care Coordinator (ECC) is responsible for managing the client experience from referral to continuity of care. The role of the ECC begins in the field, where they will assist the Operations Manager with developing and maintain relationships with referral sources such as assisted/independent living facilities, hospice providers, geriatric care managers, medical specialist and other senior-related referral sources. Next, the ECC will interact with prospective clients by taking referral calls, educating the prospective client on our services, and scheduling in-home visits. Once the client has joined, the ECC will be responsible for managing the uninterrupted care of the client through the organization of family resources and interaction with other personnel, such as community providers, insurance companies, case managers and other third-party payors. Finally, the ECC will work directly with the Internal Care Coordinator (ICC) to ensure that shifts and hours are matched with caregiver qualifications to ensure a platinum experience for our clients.
JOB REQUIREMENTS:
- Minimum of high school diploma or GED.
- Minimum of one to three years of experience in a similar position.
- Strong experience in marketing and sales with a proven track record of success.
- Demonstrated leadership skills and outstanding people management skills.
- Exceptional customer service skills and written / verbal communication.
- Willingness to spend extended periods of time in the field with driving as a requirement.
- A reliable vehicle and valid driver’s license with the ability to pass a background check (including motor vehicle records check).
WORKING ENVIRONMENT / PHYSICAL DEMANDS:
- Office environment.
- Walking, sitting, and standing with lifting limited to files and records typically not expected to exceed 5 pounds in weight.
- Extended time at a computer work screen and on the telephone.
- Extended time in motor vehicle.
