Front Desk/Referral Clerk
Location: Albany Family Medical Center
**Actual salary total compensation is based on employee benefit selections. Compensation is subject to relatable experience and is subject to fluctuate.**
Annual Compensation: Starting at $45,342.36
- Base Pay: $31,200.00
- Employer Paid Benefits: $9,150.66 (based on PPO 1500, dental, vision - employee only)
- Employer Sponsored 401k: $1,872.00 (based on a 6% employee pre-tax contribution)
- Paid holidays: $840.00
- Flexible Leave: $2,039.70

Responsibilities/Duties:
- Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
- Ensure complete and accurate registration, including patient demographic and current insurance information.
- Assemble information concerning patient's clinical background and referral needs.
- Contact review organizations and insurance companies to ensure prior approval requirements are met.
- Present necessary medical information such as history, diagnosis and prognosis.
- Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
- Review details and expectations about the referral with patients.
- Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns.
- Establish and maintain relationships with identified service providers.
- Ensure that referrals are addressed in a timely manner.
- Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.
- Other duties as assigned.

- High school diploma/GED.
- Strong customer service focus.
- Effective verbal and written communication skills.

- Organized and able to manage competing priorities.
