Family Nurse Practitioner
POSITION SUMMARY: (Full-time position Monday-Friday)
The Family Nurse Practitioner is a key member of the Integrated Health team, engaged in providing medical services and helping to define the medical direction of the organization. The position provides primary care medical services to adults to ensure the delivery of affordable quality healthcare services. The Family Nurse Practitioner works collaboratively with the Integrated Health staff to ensure clients transition seamlessly between medical and behavioral health services.
Clinical Duties
• Performs CLIA waived laboratory testing.
• Determines need for confirmatory test and prepares specimens for send out to referral facilities by preparing appropriate requisition for the reference laboratory both manually and by computer entry.
• Maintains specimen integrity by using aseptic techniques following clinical laboratory standards and department procedures.
• Assists with maintenance of send out log sheets, ensures specimens are appropriately preserved and packaged according to procedural guidelines.
• Performs appropriate quality control procedures for testing procedures and documents in accordance with departmental policies, maintaining appropriate records and files.
• Works collaboratively with team to provide primary health care to clients including: obtaining histories, performing physical examinations, diagnosing, treating and monitoring acute and chronic diseases, immunizations, ordering and interpreting diagnostic studies, prescribing medications, and providing other treatments as needed.
• Performs and records medical assessments, such as BP readings, pulse rates, temperature readings, TB testing; monitors the collection of urine specimens, etc.
• Collaborates with Behavioral Health professionals for care coordination as needed,
• Educates clients to promote positive health behaviors and disease prevention
prevention.
• Ensure clients have knowledge and access to medical services after hours for emergencies and urgent medical needs.
• Functions as a resource for the Nurses at the Residential Sites offering clinical direction and guidance.
• Takes all steps necessary to ensure accuracy of patient data, test documentation, and recordkeeping, including updating the electronic medical record with testing orders and reporting results to state, federal and other reporting agencies.
• Greets and escorts patients to the examining room in a timely manner to accomplish good patient flow. Transports patients to and from exam rooms via wheelchair when applicable.
• May assist with delivery of direct care/patient services within NAC residential and/or housing facilities
• Ensures a safe working environment by adhering to the standards set by Occupational Safety and Health Administration.
• Complies fully with all patient and data confidentiality policies and regulations, including, but not limited to HIPPA
• Provides feedback regarding the medical direction of the organization.
• Keeps abreast of emerging models in health care delivery; identify and define new and innovative strategies to achieve business goals and objectives
• Identifies opportunities to collaborate and develop clinical integration opportunities with health delivery systems to achieve affordable outcomes.
• Completion of disability certifications
• Conducting telehealth appointments as necessary
COVID-19 Testing and Laboratory Duties
• Administers Covid-19 vaccine and testing.
• Completes accompanying documentation.
• Prepares training materials and facilitates training for NAC staff who conduct COVID-19 testing
Maintain Patient Results Log and compile related data submissions as required.
• Assists with development of medical procedures/protocols for agency COVID-19 testing plan implementation including infection prevention and control.
• Provides clinical intervention, case management, care coordination, referral and contact tracing following positive COVID-19 testing for patients and family members.
Administrative Duties:
• Checking 3rd party websites for lab work and imaging results
• Function as resource for residential site medical questions
• On-call availability to residential sites for after-hours questions
• Creating, reviewing, and signing medically related policies and procedures
Supervision of nurses at residential sites
Participates in the following:
Quality Management Committee, including quality improvement and compliance
o Team meetings based on availability
o Policy and Procedure Meeting based on availability
• Works collaboratively with Integrated Clinic Medical Director
