JOB SUMMARY:
The Families First program provides intensive case management services to children with severe emotional disabilities (SED), and their families, who are at risk of psychiatric inpatient hospitalization or other out-of-home placements. The Service Facilitator incorporates the wraparound model of service delivery using multi-disciplinary treatment plans to identify needed services. The multi-disciplinary team model includes the family as equal partners, case managers as mediators, family advocates, and others selected by the family as members of the team, as well as representation from community providers (mental health / AODA / schools / juvenile justice / child welfare workers). The Service Facilitator develops and coordinates services based on families strengths as well as their needs.

KEY RESPONSIBILITIES:
- Create individual treatment plans and update as changes occur.
- Develop a family support team comprised of formal and informal supports.
- Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.
- Develop a crisis/safety plan for the client and family.
- Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
- Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other appointments that may require family support or advocacy.
- Safely transport clients as needed.
LOCATION: Office is based at the Kenosha Job Center with flexibility for hybrid/remote work scheduling. Local travel is also required to meet with clients within the home and other community spaces, with mileage reimbursement. Must be able to meet in-person with clients in Kenosha County, Wisconsin.
SCHEDULE: Monday-Friday between 8:00am-5:00pm, which occasional evenings to accommodate families' needs.

SALARY: $21.63 per hour; Plus $1,000 salary increase every six months for the first 2 years of employment!
REQUIREMENTS:
- Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
- Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related fields.
- One year experience working with the severely emotionally disabled population is preferred, but not required.
