JOBSUMMARY: As an Early InterventionService Coordinator, you will provide support to families of infants and toddlers who
have or are at risk for developmental delays in their homes and community settings. You and the Early Intervention team will
partner with parents/caregivers to ensure service delivery is individualized, meaningful for the family, and grounded in
evidence-based practice. Through this collaborative process, you prioritize families’ priorities, needs, culture, and values.

Position Responsibilities:
- Participates in home and community visits with families and teammates
- Completion of screenings, evaluations, routine-based family assessment and ongoing child and family assessments
- Participates in weekly team meetings
- Development of the Individual Family Service Plan (IFSP) and ensure timelines are followed
- Participate in conversations with families and teammates to identify a family's strengths, resources and priorities
- Completion of Child Outcome Summary (COS)
- Assist families with any transition
- Maintain necessary paperwork and records to satisfy county billing, MA billing and correspondences as necessary
- Maintain collaborative contacts with community partners and referral sources; emphasizing evidence-based practices
for infants and toddlers
Position Requirements:
- A Bachelor’s Degree with a major or concentration child development, education or special education or related
human service field
- Experience with supporting families, infants and toddlers is preferred
- Knowledge of family systems, early childhood development and supporting those with differing abilities
- Excellent written, verbal communication, and organizational skills
- Current driver’s license and reliable transportation
