Job description

Position Purpose: . Assist individuals with disabilities enjoy full, active, meaningful lives. The Home Manager will assist in directing the daily operations of the home. The Home Manager serves as a resource for all staff; their primary duties are to work hands on with direct Support Professionals, to provide supervision and support to Direct Support Professionals and persons served. Hours worked will be primarily First shift .

Job Duties/Responsibilities: . 1. Assure client and staff safety in all respects. 2. Serve as a model for staff behavior and performance. 3. Provide support to staff, and supported persons. 4. Monitor staff performance and offer feedback when needed. 5. Implement all applicable policies, procedures, and/or legal commitments. 6. Actively participate in staff meetings, workshops and special meetings. 7. Be an advocate for all supported persons. 8. Ensure all documentation is accurate and completed. 9. Assure supported persons receive medications/treatments as prescribed. 10. Assure person centered active participation. 11. Assist in supervising and transporting supported persons to community activities. 12. Know and follow emergency home procedures for fire, tornadoes, accidents, and other serious incidents affecting supported persons or Direct Support Professional. 13. Assure guardians and parents are knowledgeable and involved in the supported person’s life, based upon the desires of the supported person. 14. Help maintain the quality of home environment by taking initiative in cleaning and making minor repairs of home, and property, and grounds when required. 15. Always involve the supported person when appropriate. 16. Support persons in shopping, meal preparation, and clean-up. 17. Be a strong advocate for improving the quality of life for the supported person. 18. To treat all individuals with dignity and respect. 19. To support individuals to take control of their lives, to make wise decisions that enhances their ability to reach their goals. To support individuals to take on as much control as possible over the routines and activities in their home. 20. To teach, assist and support individuals in the community and at home so that the individual is able to learn, practice and master skills that enable him/her to enhance his/her status by assuming socially valued roles. 21. To support each individual’s existing relationships and increase their network of friends and acquaintances. 22. To insure that all individuals are supported to explore and express their interests for community membership. 23. To ensure that the health and well-being of individuals are met by accompanying individuals on appointments as needed, administering medications (or supporting them to self-medicate) in accordance with guidelines .24 Provide a well-balanced diet prepared to meet the individuals needs (when applicable), following all instructions given by the Physician and Health Care Coordinator (Program designated nurse). 25 Assist with community activities such as purchasing clothing, personal items, and visits 26. Maintain cleanliness of the home through daily, weekly, monthly cleaning duties. Including laundry, dishes, bed making, vacuuming, offering opportunities for individuals to participate, learn and increase independence. 27. Maintain all required certifications such as CPR/First Aid, medication administration. 28. To complete agency training programs in a timely manner and participate in other professional development as required by supervisor. 29. To facilitate teamwork between all team members via communication, sharing of responsibilities and a positive, constructive attitude. 30.Assists persons served at the time of admission with personal items and helps familiarize them to the home. 31. Assists in carrying out planned activities. 32.Assures that all duties are completed and documented, as well as unusual incidents involving consumers, staff or other persons affecting the home and including seizures, accidents, or other medical emergencies. 33.The Home Manager the responsibility for being on call after normal business hours unless other arrangements have been made, a call log is required for all time spent responding to staff and consumer concerns. 34.Completes other job duties as directed by supervisor. Must accept risk of working in a group setting where employees are exposed to communicable disease and occasional combativeness. 4. Must demonstrate good judgment and the ability to choose the best course of action when needed. 5. Must possess a good attendance record. Must be able to meet physical demands including stooping, turning, stretching and lifting. Good physical health, to include a negative reaction to a TB test. 6. Must possess good visual and audio acuity necessary to detect changes in the person served condition. 7. Must have completed training programs required by the Department of Community Health, Department of Human Services and/or the local Community Mental Health Agency. . . Physical Requirements . Able to lift up to 50lbs. Bending, Lifting, Reaching, Squatting and Stooping. Able to Stand and Utilize a Step Ladder. Able to Assist in Daily Personal Care Needs'
