Job Order Hourly Rate of Pay: $22.55 (Based on education and/or years of relevant work experience reflected on the application)
This temporary position will be assigned to the Department of Health and Human Services -PH - W&C - CDSA.
This position is part of a team that writes, develops individualized Service plans and evaluates the child's progress, and adjusts plans as needed. Early Intervention Service Coordinators provide in-home or center-based consultation services and are responsible for utilizing a variety of formalized assessment tools for the child and/or other family members. Also responsible for coordinating with other disciplines and agencies, EISCs ensure a comprehensive assessment and reviews of family resources, relationships, and attitudes, making independent clinical judgments regarding appropriate services to be provided to clients and families. EISC s establish both short and long-range client and family-centered goals and develop, negotiate, implement, monitor, and adjust individualized family service plans (IFSPs) and coordinate specialized services such as physical, occupational, psychological, and speech therapies, as needed and based on family choice.
