Title: Service Coordinator
Location: Early Intervention Office
About the Role:
We are seeking a motivated and dedicated individual to join our team as a Service Coordinator. The Service Coordinator will be responsible for coordinating and implementing services for families with young children with developmental delays or disabilities. The successful candidate will have excellent organizational and communication skills, and a commitment to making a positive impact in the lives of young children and their families.
Responsibilities:
- Demonstrate knowledge and understanding in the following areas:
- Infants and toddlers who may be eligible for early intervention services
- State and federal laws and regulations pertaining to the Early Intervention Program
- Principles of family centered services
- The nature and scope of services available under the Early Intervention Program and the system of payments for services in the State
- Assess the needs of families and children with developmental delays or disabilities, and develop individualized service plans.
- Coordinate and provide case management services to families, including referrals to community resources, service coordination, and advocacy.
- Communicate regularly with families, service providers, and team members to ensure high-quality services and support.
- Maintain accurate and up-to-date records of services provided and progress towards individualized goals.
- Attend and participate in team meetings, trainings, and other professional development opportunities.
- Collaborate with interdisciplinary teams, including speech therapists, occupational therapists, and educational professionals, to ensure comprehensive and coordinated services.
Qualifications:
A minimum of one of the following educational or service coordination credentials:
- A bachelor's degree in a health or human service field
- Knowledge of early childhood development, developmental disabilities, and community resources.
- Strong organizational and communication skills, with the ability to effectively manage multiple tasks and prioritize responsibilities.
- Ability to work independently and as part of a team.
- Passion for making a positive impact in the lives of young children and their families.
- Two years of experience in service coordination activities as delineated in regulation (voluntary or part-time experience which can be verified will be accepted on a pro rata status)
- One year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities
- One year of service coordination experience and an associates degree in a health or human service field
