$18.45-$22.41/hour - Placement in the range will be determined based on experience and other factors allowed by law.
GENERAL SUMMARY
Provides employability and Medicaid Disability determinations, facilitates medical documentation requests, and processes SSI/SSD applications according to contracts, to DSS applicants and recipients to enhance self-sufficiency and decrease dependence on public assistance.
COMPANY STANDARDS
- Maintains regular attendance and punctuality in order to provide services to individuals served This position works at an offsite location and performs all hours on site unless required by business needs and approved.
- Follows all safety rules and regulations; wears and/or uses required protective equipment while working; immediately reports any safety concerns or unsafe working conditions to Employment Services management.
- Conducts all aspects of job responsibilities in compliance with the JM Murray mission statement, policies and procedures, professional standards, and applicable governmental laws, rules, and regulations.
ESSENTIAL FUNCTIONS
- Interview referred individuals to outline anecdotal history. Provide employment recommendations to involved agencies.
- Establish relationship with medical providers in order to discuss treatment plan, prognosis, and determination of disability / employability.
- Assist clients with SSI/SSD application process and denial hearing requests as designated on a County-wide basis.
- Coordinate with referral source and recipient to ensure appropriate enrollment into program activities.
- Complete Medicaid Disability Determinations and gather all documentation necessary for approval.
- Establish and maintain a positive working relationship with referral source and coordinate individual’s medical determination and program needs.
- Maintain a case record for each participant to include historical medical and/or counseling material and case notes to document all services provided.
- Initiate problem resolution of issues that arise and ensure that supervisor and DSS are kept informed.
- Prepare and ensure completion of monthly/quarterly/annual reports required by contract agencies to reflect enrollments, status, referrals, etc.
- Perform clerical duties necessary to maintain accurate records and service operations including but not limited to filing, copying, computer entry, etc.
- Attend all training and meetings as requested/required by supervisor.
- Conducts self in a manner as to always contribute to maximum individual growth and in accordance with all regulations, the company Code of Ethics and Company policy and procedures. Represents JM Murray and the JM Murray mission in a positive manner, treating people with respect and dignity.

EDUCATION and EXPERIENCE
Bachelor's degree is preferred; Associate’s degree with 1-3 years of experience in vocational services, disabilities services, business, personnel management, mental health or social services counseling required
KNOWLEDGE and SKILLS
Ability to establish rapport with different individuals and personalities, and build trust
Ability to provide services to individuals and in small groups
Active Listening skills
Problem solving and trouble shooting skills
Good written communication skills and efficient use of technology
Flexibility to adjust to changes in priorities and work tasks
Knowledge of SSI/SSD benefits and the application process is a plus
OTHER REQUIREMENTS
Requires clearance through a Background Check process that will include, but is not limited to,
clearance through the NYS Justice Center (Office of Mental Health).
WORKING CONDITIONS
Job assignments may take place indoors and outdoors and require travel to various community locations, in various environmental conditions including rain, snow and cold weather.
