As the Director of Nursing (DON) at CareAparent, your role is pivotal in the comprehensive management and advancement of our home care services. Reporting directly to the SVP of Operations, you will spearhead clinical strategic initiatives that actively foster the development, implementation, and oversight of both established and new programs, ensuring alignment with our core values, mission, and vision.
Within your leadership portfolio, you will directly manage clinical manager(s), RN case managers, RN Nurse Educator, and others as assigned. Your responsibilities encompass crafting strategic plans and budgets, driving program development and implementation, and maintaining unwavering adherence to Quality Assurance and Performance Improvement (QAPI) and compliance standards. Additionally, you will collaborate with the SVP of QAPI and Compliance in navigating audits from The Joint Commission, CMS, and State(s) where our services are extended.

Collaborating with both the SVPs of Operations and QAPI and Compliance, you will contribute to refining processes and policies governing the administration and delivery of skilled care, fostering an environment of continuous improvement.
In addition to leadership responsibilities, you will diligently monitor and report on weekly, monthly, quarterly, and annual Key Performance Indicators (KPIs), providing invaluable insights to the leadership team. Through meticulous planning, daily quality assurance measures, and exemplary leadership, you will play a pivotal role in contributing to the ongoing success of our home health services. Your dedication will be evident in the achievement of meeting the Quadruple Aim (Client Engagement, Employee Engagement, Clinical Outcomes, and Fiscal Responsibility), ultimately leaving a positive and enduring impact on the lives of our valued clients and employees.
Job Duties and Responsibilities:
- Embody and enforce CareAparent's (CAP's) vision, mission, and core values.
- Embrace leading and managing others with a servant heart.
- Collaborate with the Executive Leadership Team (ELT) in designing and implementing CAP's annual strategic plan.
- Develop and manage department's budget.
- Oversee clinical team productivity, utilization, QA, predicting staffing requirements, and collaborating with HR on recruiting and retention initiatives to reach team KPIs.
- Manage weekly, monthly, quarterly, and annual clinical performance KPIs (Quadruple Aim), report regularly to ELT.
- Develop and mentor team members through continuous improvement and overall successful performance.
- Manage and monitor direct reports performance guiding them towards successful KPIs.
- Collaborate with SVP of QAPI and Compliance ensuring successful adherence to QAPI and compliance initiatives.
- Assist organization through compliance audits from internal audits to The Joint Commission, CMS, and State(s) we provide services in.
- Design, implement, and manage clinical programs (e.g. Falls Prevention, Pain Management, Dementia).
- Monitor infection control and safety practices and standards for clinical team members.
- Collaborate with Client Services Team on client admissions, referral contacts, and pending referrals, ensuring effective communication with both internal and external partners.
- Perform other related leadership duties as assigned by ELT.
