Job Description:SUMMARY OF POSITION RESPONSIBILITIES: The role of the Direct Care staff person is to provide the programs clients with supportive services, supervision, personalized assistance, health-related services, or a combination of these services to meet the needs of individuals who are unable to perform, or who need assistance in performing, the activities of daily living or instrumental activities of daily living, in a way that promotes optimum dignity and independence for the individuals. COMMON MISCONCEPTIONS WHICH MAY REQUIRE CLARIFICATION: - So long a resident is in the facility in which the Direct Care staff is assigned, the Direct Care staff must be present.- Regardless of the time-of-day services are rendered, the Direct Care staff is to remain awake.- The Direct Care staff person may not leave the assigned site during the service time block until all services are rendered at specified times herein. Responsibilities and Competencies (all shifts) - manage the day-to-day housing operations and property management needs on the property in which your group living clients reside- assist assigned clients with ADLs and IADLs as needed and indicated on the service plan- attend staff meetings once per week or as scheduled- Responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's service plan promoting independence, freedom of choice and overall wellness- Must collaborate with the client's direct support worker therapist, nurse case manager, mental health case manager, psychiatrist and other persons involved in the client's care to ensure the client complies with treatment schedules on an as needed basis- Be available to clients in the event of a mental health crisis while on duty and after hours- Carry out any other work related functions requested by the supervisor- Ensure clients have an adequate supply of medications and monitor medications on a regular basis for each client- Document incidents, hospital admissions in contact note and incident report- conduct room and facility inspections for all assigned areas 3 times per day while on duty: upon arriving on duty, half-way through your shift, and just before leaving duty- Participate in team meetings as scheduled- you may need to transport clients in company vehicle- You will collaborate regarding your schedule with the Rehabilitation Specialist and ensure you complete the following additional duties:(a) manage the tele-medicine system when clients in your caseload are scheduled to receive treatment via tele-medicine with OMHC providers; (b) collect urine/saliva samples from clients for medication and substance abuse monitoring as needed; (c) be available during community housing unit inspections and ensure clients manage housing units according to posted policy; (d) receive food and supply deliveries; (e) assist registered nurse case manager; (f) collaborate with client's therapist and all persons involved in the client's treatment at least monthly- Are responsible for implementing the rehabilitation activities outlined in the adult or minor's IRP.- If assigned on an hourly basis in an assisted living facility, also serve in the capacity of "Direct Care Staff" while on duty.- If assigned on an hourly basis in a residential treatment facility or supportive housing facility, also serve in the capacity of "House Manager" while on duty.- Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by supervisor.- Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.- Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.- Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation.- Support the maintenance of a safe environment by participating in drills as requested.- Advise the supervisor on the development and implementation of protocols to better enhance the efficiency of day to day operations effecting change when approvals are made.- Troubleshoot and abate any issues that could adversely affect the day-to-day operations.- Report to the Supervisor daily and as requested. Report deviation of operational standards to CEO daily.- Collaborate with department leaders, external landlords, vendors, utilities providers, housing related external entities and supervisees regularly as needed.- Have knowledge of Policies and Procedures of Holbrock Estates Supportive Housing / Lighthouse Behavioral Healthcare.- Maintain a working knowledge of COMAR as it pertains to assisted living and other residential regulations within the scope of services rendered by the company.- Report to the ALM, CEO, COO, manager via email when starting and ending service period if not using Company’s time keeping system.- Check emails and company group texts at least every 30 minutes while on duty; respond accordingly.- Assist the ALM, CEO, COO and/or HR with other services as requested / directed. Responsibilities and Competencies (1st shift 7:00 AM – 4:00 PM) - Collaborate with staff leaving duty, complete staff collaboration form and submit to supervisor- Ensure all clients receive breakfast in the dining area and document clients’ food intake in micromanager software. Notify the supervisor if there is not adequate supply of food for 72 hours. Wash dishes and clean the dining area. *if no dietary staff present, prepare the meal or assist the client with meal preparation *if dietary staff are present, serve the meal once prepared by dietary staff- Ensure all clients receive morning medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.- Review with each client the client's treatment, services and activity schedule for the day. Update the clients calendar and information board calendar as needed. Issue 3-ply appointment slips for new appointments. Schedule / coordinate transportation for clients appointments today with logistics coordinator / supervisor / UberHealth / MTA mobility / client's preferred method of travel.- Write care notes on each client residing in the facility in icaremanager. Update the resident status on clients if clients are on LOA, hospitalized or no longer reside on site.- Verify in icaremanager under resident agreement section that each client has a resident agreement on file. If a resident agreement is not on file, send an email to housing@upliftincmd.org and notify the supervisor.- Verify in icaremanager under RAT section that each client has a RAT done by the delegating nurse within the past 45 days prior to today's date. If there is no RAT or the RAT is outdated, send an email to nursing@upliftincmd.org and notify the supervisor.- Verify in icaremanager under service plan section that each client has a service plan done by the delegating nurse within the past 6 months days prior to today's date. If there is no service plan or the service plan is outdated, send an email to nursing@upliftincmd.org and notify the supervisor.- Be available for the emergent needs of the clients answering questions and troubleshooting as needed- Ensure all clients receive lunch in the dining area and document clients’ food intake in icaremanager software. Notify the supervisor if there is not an adequate supply of food for 72 hours. Wash dishes and clean the dining area. *if no dietary staff present, prepare the meal or assist the client with meal preparation *if dietary staff are present, serve the meal once prepared by dietary staff- Ensure all clients receive afternoon medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.- Review with each client the clients treatment, services and activity schedule for the day. Update the clients calendar and information board calendar as needed. Issue 3-ply appointment slips for new appointments. Schedule / coordinate transportation for clients appointments today with logistics coordinator / supervisor / UberHealth / MTA mobility / client's preferred method of travel.- Verify in icaremanager under fire drill section that your facility fire drills are up to date. That means on both shifts, a fire drill has been completed for each quarter. Shift A is defined as the time period between 7:00 AM to 7:00 PM. Shift B is defined as the time period between 7:00 PM to 7:00 AM. Quarter 1 is defined as January to March. Quarter 2 is defined April to June. Quarter 3 is defined as July to September. Quarter 4 is defined as October to December. If the fire drills are not compliant send an email to housing@upliftincmd.org detailing the SHIFT and QUARTER missing then notify the supervisor.- Verify in icaremanager under disaster drill section that your facility disaster drills are up to date. That means on both shifts, a disaster drill has been completed for each semi-annual period. Shift A is defined as the time period between 7:00 AM to 7:00 PM. Shift B is defined as the time period between 7:00 PM to 7:00 AM. Semi-annual period 1 is defined as January to June. Semi-annual period 2 is defined as July to December. If the disaster drills are not compliant send an email to housing@upliftincmd.org detailing the SHIFT and SEMI-ANNUAL PERIOD missing then notify the supervisor.- Collaborate with staff entering duty, complete staff collaboration form and submit to supervisor- Check out with supervisor Responsibilities and Competencies (2nd shift 3:00 PM – 12:00 AM) - Collaborate with staff leaving duty, complete staff collaboration form and submit to supervisor- Ensure bathroom, bedrooms, kitchen and common areas clean and sanitized - at minimum: spray mattress, linens and pillows with lysol and bed bug spray, clean touchables with bleach/lysol, sweep/mop floors, spray chairs/sofas with lysol and bed bug spray- Be available for the emergent needs of the clients answering questions and troubleshooting as needed- Perform any work related task requested by the supervisor and enforce policies and procedures. Notify the supervisor if any visitor comes to the site prior to allowing the visitor to enter. (Activities, servicing the clients spiritual needs, and/or if applicable food/supply ordering may be done at this time at the request of the supervisor)- Ensure all clients receive dinner and evening snack* in the dining area and document clients’ food intake in icaremanager software. Notify the supervisor if there is not an adequate supply of food for 72 hours. Wash dishes and clean the dining area.- *if no dietary staff present, prepare the meal or assist the client with meal preparation- *if dietary staff are present, serve the meal once prepared by dietary staff- Ensure all clients receive evening and bedtime* medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.- Walk-thru facility and ensure: ___menu is current and posted, ___ licenses are current and posted,___ all equipment/furniture is in tact, ___there are no pests or hazards, ___ activity schedule is current and posted, ___refrigerator/freezer thermometer are reading correct temperature ranges, ____each resident has all the furnishings and items they require. For any missing items in the above list, send an email to housing@upliftincmd.org and notify the supervisor.- Ensure all clients receive evening snacks in the dining area and document clients’ food intake in icaremanager software. Notify the supervisor if there is not an adequate supply of food for 72 hours. Wash dishes and clean the dining area.- *if no dietary staff present, prepare the meal or assist the client with meal preparation- *if dietary staff are present, serve the meal once prepared by dietary staff- Ensure all clients receive bedtime medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.- Review with each awake/active client the clients treatment, services and activity schedule for the day. Update the clients calendar and information board calendar as needed. Issue 3-ply appointment slips for new appointments. Schedule / coordinate transportation for clients appointments today with logistics coordinator / supervisor / UberHealth / MTA mobility / client's preferred method of travel.- Write care notes on each client residing in the facility in icaremanager. Update the resident status on clients if clients are on LOA, hospitalized or no longer reside on site.- Verify in icaremanager under resident agreement section that each client has a resident agreement on file. If there is no service plan or the service plan is outdated, send an email to nursing@upliftincmd.org and notify the supervisor.- Be available for the emergent needs of the clients answering questions and troubleshooting as needed- Ensure all visitors are out of the building in accordance with the visitor policy- Collaborate with staff entering duty, complete staff collaboration form and submit to supervisor- Check out with supervisor Responsibilities and Competencies (3rd shift 11:00 PM – 8:00 AM) - Collaborate with staff leaving duty, complete staff collaboration form and submit to supervisor- Ensure the facility is secure and all clients are in by established curfew times. Check all late returning clients for non-compliant paraphernalia. Report issues to the supervisor.- Review each client's treatment, services and activity schedule for the upcoming day. Update the clients calendar and information board calendar as needed. Issue 3-ply appointment slips for new appointments.
