SUMMARY OF POSITION RESPONSIBILITIES:


Under the supervision of the Psychiatric Rehabilitation Specialist, growing Psychiatric Rehabilitation Program is seeking qualified and competent Family Care Coordinators (Direct CarePsychiatric Rehabilitation workers) to service clients in the community on an independently contract basis. Our Psychiatric Rehabilitation Program (PRP) provides all persons and family systems, community-based assistance to optimize their overall wellness and quality of life with the goal of rehabilitation and recovery. Our organization promotes the healthy restoration of the individual’s deficiencies by assisting them in the development of their life skills. CLH’s primary focus is to empower the client and assist the individual to identify personal goals and overcome objectives and work towards a full recovery. Our strengths-based rehabilitation services are designed to produce healing and restoration in communities. A Family Care Coordinator is expected to facilitate Psychiatric Rehabilitation, behavioral support services, and crisis intervention for assigned clients, according to Individualized Rehabilitation Plan, by:


  • teaching and instruction
  • performing life skills training
  • encouragement of positive change
  • skill demonstrations and practice
  • promoting individual wellness, self-management, and recovery
  • assisting in the restoration and/or improvement of self-care social skill
  • assisting with access to entitlements
  • facilitating effective strategies for strengthened independent Living and the achievement of self-sufficiency goals
  • Exposing individuals to academic, vocational, and employment training and resources
  • Facilitating culturally relevant activities directed toward improved social relationships
  • Encouraging Health Promotion, Proper Nutrition, and Exercise. 


ESSENTIAL DUTIES :

  • Provide PRP services to assigned clients as scheduled by the Company (supervisor). Must document each encounter in the company's electronic health record on the same day of the encounter by the close of business. Worker may set schedule to meet each client independently.
  • Attend staff meetings once per week or as scheduled
  • Responsible for carrying out services and supports that meet the client's medically necessary mental health needs reflected in the client's individual rehabilitation plan (IRP) promoting independence, freedom of choice and overall wellness
  • Must collaborate with the client's therapist, psychiatric nurse practitioner and other persons involved in the client's care to ensure the client complies with treatment schedules on a weekly or monthly basis.
  • Assist the client with monitoring and obtaining access to all entitlement benefits for which the client may be eligible and monitor benefit status on a monthly basis helping the client to apply when necessary
  • Be available to clients in the event of a mental health crisis while on duty and after hours
  • Carry out any other work related functions requested by the supervisor
  • Are responsible for implementing the rehabilitation activities outlined in the adult or minor's IRP.
  • Participate in external and internal audits/surveys (JCAHO/CSA/OHCQ) as directed by supervisor.
  • Participate in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.
  • Responsible for following regulations of COMAR, JCAHO, any other regulatory body, and company policies/ procedures related to service delivery and documentation.
  • Advise the supervisor on the development and implementation of protocols to better enhance the efficiency of day to day operations effecting change when approvals are made.
  • Troubleshoot and abate any issues that could adversely affect the day-to-day operations.
  • Report to the Supervisor daily and as requested. Report deviation of operational standards to CEO daily.
  • Perform Blended Services for all consumers – Require Onsite (Office Visit) & Offsite (Home/Community Visit)    


OTHER REQUIREMENTS:


    • High School Diploma/GED; AA Degree or greater preferred.
    • If servicing minors: 1. A bachelor's degree in a health-related field; 2. An associate's degree in a health-related field; 3. 30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; or 4. 1 year of work experience in a supervised mental health setting.
    • Minimum two (2+) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders.  These will be verified through your professional references provided.
    • Ability to apply the concepts of customer service in the workplace. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.
    • Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills
    • Ability to handle multiple tasks and meet critical deadlines.  Ability to meet short and long-term goals and project deadlines.
    • Respect for the confidentiality of all communications required within job duties, including client/employee records and documents, and electronic transmission by voice, data, and wire
    • Must demonstrate you are free from TB
    • Must pass a criminal background check and have no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff
    • Must have a driver's license and your own reliable transportation
    • Must be 18 years or older
    • Once hired, must complete 40 hours specialized PRP training before servicing clients independently and an additional 20 hours direct supervision if rendering services to minors
    • Current first aid and CPR certification
    • Ability to follow CLH Clinic practice guidelines and policies
    • MUST BE COVID VACCINATED or at least have your first dose by start date with the 2nd dose scheduled and documented.
