PRP Direct Care Staff/Rehab Counselor

Prince Healthcare Services Inc., a Psychiatric Rehabilitation Program in Pasadena, MD., is hiring PRP Direct Care Staff/Case worker

Position Overview
Responsible for assessing client’s needs and capabilities, providing supported living services, supportive counseling, skills enhancement and crisis intervention to facilitate progress towards rehabilitation plan goals and integration into community life.

Specific Responsibilities

• 1. Face–to-face contact with clients as planned for by the rehabilitation team (PRP worker and Rehab Specialist) and client.
• Provide a minimum of 6 or 4 hours of direct service, per client, per month
• Complete daily contact sheets/notes, monthly progress notes, report and other notes for each client in ICANOTES
• Provide supportive services that assist consumers in maintaining a safe, healthy lifestyle and living environment and promote and encourage meaningful community participation.
• Transport, accompany, or assist client with getting to appointment(s) as necessary
• Participation in individual IRP goal development and provision of support and assistance to facilitate client working towards and accomplishing their goals.
• Serve as liaison between clients and mental health professionals, client’s family, therapist, community agencies, housing providers, and others involved in the process of problem resolution and on-going case management.
• Perform crisis intervention and assistance as necessary
• Monitor adequacy of, and assist clients in accessing and managing entitlements
• Maintain client’s charts and documentation of all services in a manner consistent with agency policies and COMAR regulations
• Perform administrative tasks as required
• Develop and maintain relationships with local service providers to promote coordination and integration of care
• Other duties as assigned

· Research and provide resources to your clients
· Notify supervisor of all crisis situations to help with decision making

Experience:

• PRP: 1 year (required)
• ICANOTES: (A Plus)
• Proficiency in MS Office 365

Job Types: Full-time, Part-time, Contract

Pay: Up to $30.00 per hour

Expected hours: No less than 6 per week
