Description
Serves as patient advocate to help clients navigate the dental system and access needed follow-up care. Interacts with patients and their family to explain dental terminology and procedures, research inquiries, instruct on how to use home health care products, and may collect and prepare data for further research.
Essential Functions

  • Provides a linkage between clinics, individual clients and the community network of service providers to increase effective collaboration and use of resources.
  • In coordination with pediatric care team (dental hygienist and/or dentist), provides direct assistance to clients seeking care, including arranging other appointments, assisting with insurance or arranging transportation.
  • Seeks to understand and meet the needs of customers (patients, families, vendors, co-workers, etc.) through respectful, courteous and culturally sensitive interactions.
  • Serves as the Bright Smiles Program (pediatric dental program) information resource contact for patient and clinic needs.
  • Provides dental education for patients and their families individually and in a group setting.
  • Supports staff and patient scheduling workflows, program data collection and tracking.
  • Facilitates follow up appointments that match the patient need to the most appropriate resource as requested by care team.
  • Provides follow-up on referral appointments to ensure completeness and additional visits per staff request.
  • Provides information and referrals to appropriate community agencies and resources.
  • Participates in oral health education/outreach activities and program capacity building activities.
  • Performs other related duties as assigned.

Supervisory Responsibilities
This position does not supervise other staff but may provide work direction as needed to volunteers, students or new employees.
Work Environment & Physical Demands
This position is very active and requires standing, and/or walking, bending, kneeling, and stooping. The employee may need to lift and/or move items over 20 pounds. This role may come into contact with patients who may have contagious illnesses.
Position Type and Expected Hours of Work
Dental clinic environment with regular assigned hours may include evening and/or weekend hours.
Travel Requirements
Travel between dental clinic is expected

Qualifications
Required Education and Experience
  • High school or equivalent.
  • Driver's license/car required.
  • Proof of COVID Vaccination Required, Religious and Medical exemption will be considered

Preferred Education and Experience
  • One year of previous outreach, case management, health education or clinical experience.
  • Bilingual verbal and written English/Spanish English/Hmong highly preferred
  • Associate degree in related field.
  • Knowledge of dental health care systems.
  • Pediatric experience
  • Ability to establish good rapport with patients of various socio-economic and ethnic backgrounds.
  • Skills in establishing and maintaining effective working relationships with staff and the community.
