SUMMARY:The Data Entry Clerk assists with the overall needs of the Patient Records Department and takes a lead role in filing and purging of records, Data Entry of daily inpatient ESM enrollment and disenrollment forms. This position works independently or as part of a Medical Records department and must have the ability to lift and carry 25 pounds of records walking up and down stairs and from one area to another.
ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:- Participate in Bureau of Substance Addiction Services (BSAS) ESM training.
- Successfully complete BSAS ESM test.
- Review patient medical record for data required for reporting.
- Enter ESM enrollment and disenrollment data into state portal on daily basis.
- Participate in quality improvement studies as assigned.
- Maintain confidentiality of information.
- Ability to reach, bend and stoop to remove and replace filed records.
- Ability to maintain pleasant professional manner and demeanor in personal interactions and in the handling of telephone inquiries from patients and other interested parties.
- Ability to correctly file numerically and alphabetically.
- Able to carry out verbal and written instructions and to communicate clearly and effectively in spoken and written form.
- Perform all other duties as assigned.

MINIMUM KNOWLEDGE, Experience, AND SKILLS REQUIRED:
The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job.
- Education: High School Diploma or comparable experience working with medical records.
- Experience: Computer proficient with the ability to perform computer Data Entry accurately and ability to learn new computer applications. Prior experience in auditing, managing, and maintaining patient records preferred.
- Skills: Knowledge of computer programs used to maintain patient record data.
