This position serves the northern New Jersey area and will require working remotely from various locations in
Essex, Sussex, and Bergen counties, New Jersey.
The Community Smiles Outreach Coordinator is responsible for coordinating with clients to perform dental wellness education and oral health screenings in compliance with state regulations with a goal to improve LIBERTY members’ oral health and access to care. This position may perform some clinical activities under general supervision.
Job Summary Details:
- Provide outreach to LIBERTY Dental Plan members.
- Educate and coordinate care for members on the Dental Wellness Plan benefits and oral health practices.
- Perform oral health screenings at various health plan events such as member meetings, health fairs, etc.
- Perform oral health screenings at day care centers, elementary and middle schools, etc.
- Obtain the necessary consent forms from parents/guardians to conduct the oral health screenings referenced above.
- Build relationships with individuals and organizations to facilitate coordination with community-based resources, as well as medical and dental providers.
- Work with community-based services and representatives to educate and assist program members about behaviors that can enhance their oral health, as well as their overall health, and successfully navigate program services.
- Develop and maintain relationships with key individuals to promote the Dental Wellness Plan.
- Work with leadership and other internal stakeholders to develop and distribute internal and external communications for the program with all outlets (social media, print and television news; stakeholders, etc.).
- Collaborate with customer service representatives to maintain effective communication with members.
- Collaborate with data analysis staff to develop and maintain a report system for outcomes.
- Collaborate with Member and Provider Services staff to communicate member issues requiring interventions to appropriate internal departments and providers.
- Maintains confidentiality and uses only the minimum amount of protected health information (PHI) necessary to accomplish job-related responsibilities.
- Other duties as assigned.
Education/Experience:
- Associate Degree in dental hygiene, expanded functions dental assistant (EFDA), or equivalent combination of education and experience
- Dental hygiene license, strongly preferred, and must be currently registered and in good standing
- Ability to pass credentialing by LIBERTY Dental Plan
- 2 years' Dental healthcare experience, dental office management, and/or dental insurance background
Specific Skills/Knowledge:
- Advanced knowledge of dental terminology
- Bilingual with demonstrated ability to read, write and speak non-English language fluently
- Ability to collaborate and build coalition
- Project management skills
- Strong interpersonal and communication skills, both verbal and written
- Proficient in use of Microsoft Office applications (Word, Excel, PowerPoint)

- Must be available to work a flexible schedule including nights and weekends, typically consisting of two weekend days a month
- Knowledge and experience in working with diverse communities
- Knowledge of government structures, agencies and policies which impact authority business and progress
- Recent and related outreach experience working with providers and members, OR government program managed care product experience
- Recent and related experience working with HEDIS measures, preferred
- Must possess a valid driver’s license and proof of insurance with minimum liability of $100,000 of coverage.

Compensation:
In the spirit of pay transparency, we are excited to share the base salary for this position is $76,389 - $99,305, exclusive of fringe benefits or potential bonuses. If you are hired at LIBERTY, your final base salary compensation will be determined based on factors such as geographic location, skills, education, and/or experience. In addition to those factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any, final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical in order to allow for future & continued salary growth.
