This position is Remote, however; candidate will be required to reside in and travel throughout Thurston, Mason, and Lewis Counties

Job Summary

The Community Health Worker serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

Essential Functions

  • Engages high risk CHPW members who have been identified through data analysis and referral sources to likely benefit from Community Health Worker services.
  • Utilizes Internet databases, electronic health record systems, and other resources to locate potential eligible Member contact information.
  • Locates members that CHPW has been unable to reach by phone. This includes but is not limited to, locating members in the field connecting with collateral contacts in the health care and social services systems, reviewing pharmacy information, attempting to locate member at last known address.
  • Conducts outreach to identified clients by field visit attempts at member’s home, inpatient medical or psychiatric hospitals,, skilled nursing facility, adult living facility, shelters or homeless encampments or in a community setting. Members may also be reached via telephone or by mail, email or in some cases text.
  • Provides follow up services via face to face or telephonic with members and service planning partners as needed to coordinate reminder calls, medication and medical appointments.
  • Completes Health Assessments with members.
  • Consults or co-manages cases with the internal Care Management team members in securing and identifying needed referrals to community and network medical, behavioral health and social assistance providers through telephonic and/or face to face outreach.
  • Completes community outreach to ensure social determinant of health needs are assessed for members including but not limited to transportation services, food stamp/basic food, accessing or obtaining food boxes from food banks, attending provider appointments with members, obtaining durable medical equipment, connecting to housing resources including homelessness prevention resources, utility assistance, accessing low-cost cellphones, educating enrollees on healthy behaviors, and providing information on community resources.
  • Refers members to appropriate provider(s) and support services that promote socialization, recovery, self-advocacy, development of natural supports, and maintenance of community living skills.
  • Provides information to increase the enrollee's knowledge about his or her health conditions and improve adherence to prescribed treatment.
  • Participates in Care Management Rounds and work with an interdisciplinary team to staff cases.
  • Provides oral and/or written status updates regarding client alerts, progress and needs to responsible care managers and providers, legal mandate, or other care plan affiliates as needed to assist the program and enrollees.
    • Ensures case documentation is consistent with policies and procedures. Expands knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members
    • Partners with Community Health Clinics to meet the members needs and respond to SDOH referrals.
    • This position requires traveling on behalf of the Company and working in the field at least 75% of the time. It is essential that a current driver’s license, proof of insurance and an acceptable driving record are maintained.
    • Non-Exempt - Reporting to work on time and for all scheduled shifts is essential to this position.

  • Ensures that Supervisor, Community Support Services is made aware of any potential risk management issues in a timely manner.
  • Adhere to field safety protocols per CHPW policy and procedures.
QualificationsEducation
  • High school diploma or equivalent required; Bachelor’s Degree preferred.

License, Certifications, and other Specialized Training

  • Certification as Peer Specialist may substitute for minimum experience and education requirements.
  • Valid state issued drivers’ license required
Experience
  • Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness.
  • Experience and familiarity with at-risk populations, communities of color and local resources for persons with behavioral and/or physical health needs.

    • Bilingual English/Spanish or another second language desired.

    • Proficient with Microsoft Word, Excel and Outlook.
    • Experience using a health plan care management system or electronic medical record system.
    • Works well with others in a collaborative and respectful manner.
    • Able to multi-task, deal with complexity on a frequent basis.
    • Performs all functions of the job accurately and in a timely manner.
    • Able to work under pressure and time constraints.

    As part of our hiring process, the following criteria must be met:

    • Complete and successfully pass a criminal background check.

    Compensation and Benefits:

    The position is FLSA Non-Exempt and is not eligible for overtime. Based on market data, this position grade is 42 (see full range below) and has a 5% annual incentive target based on company, department, and individual performance goals.

    • Extended periods of sitting, computer use, talking, and possibly standing.
    • Simple grasp, firm grasp, fine manipulation, pinch, finger dexterity, supination/pronation, wrist flexion.
    • Frequent torso/back static position; occasional stooping, bending, and twisting.

    Mental:

    • Frequent decision-making. Ability to gather and assess data, determine appropriate actions, apply protocols and knowledge to unique situations, problem-solve and provide consultation.

    Work Environment:

    Office environment Employees who frequently work in front of computer monitors are at risk for environmental exposure to low-grade radiation.
