Description

SNAP is seeking qualified individuals for two Community Health Worker II positions to work in SNAP’s Essential Connections Core. These positions are full-time and open to all qualified individuals.

Starting salary for this position is $18.94-$21.30 per hour DOE

Full salary range is $18.94- $30.32

Position Summary:  This Community Health Worker II position will be focused on providing culturally appropriate health education and information, conducting outreach, building relationships with referral sources, coordinating care and helping clients navigate the health and social determinants of health systems.

Principle Duties and Responsibilities:

  1. Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.
  2. Responsible for safely transporting clients to and from appointments in a timely manner in SNAP owned vehicles, (transporting clients in personal vehicles is strictly prohibited).
  3. Perform pre-trip vehicle safety inspections.
  4. Assist clients to and from, as well as, in and out of vehicle, as needed.
  5. Operate wheelchair ramp and securement devices appropriately, as needed.
  6. Accurately record mileage.
  7. Immediately report to supervisor any problems or concerns related to vehicle.
  8. Immediately report to supervisor any problems encountered or concerns observed while transporting clients.
  9. Maintain vehicle cleanliness with daily cleaning of the interior of the vehicle and weekly cleaning of the exterior of the vehicle.
  10. Gas vehicles up as needed to keep a minimum of ¼ tank.
  11. Manage assigned caseload of clients.
  12. Work with clients to assess and prioritize needs, formulate plans to address needs, implement plans and evaluate plans.
  13. Follow-up with clients from initial identification through closure, via phone calls, home visits and visits to other sites where clients can be found.
  14. Record client care management information in the agreed upon client database (training provided) and other software no later than 24 hours after client contact.
  15. Work closely with other community agencies, medical providers, behavioral health providers etc. to help ensure that clients have comprehensive and coordinated care.
  16. Be knowledgeable about community resources appropriate to needs of clients/families.
  17. Provide referrals for services to community agencies as appropriate.
  18. Act as a client advocate and liaison between the client/family and other agencies/ service providers.
  19. Assist clients with completing applications for services. (i.e.: food benefits, SSI, TANF, GAU, housing applications, etc.)
  20. Attend regular staff meetings and other meetings as requested.
  21. Attend quarterly trainings related to health specific topics.
  22. Work with coworkers and volunteers to promote a positive, respectful and productive working environment, while protecting client confidentiality and dignity.
  23. Within the context of the stated principle job duties, serve as an ambassador of SNAP’s mission and values.
  24. Complete assigned projects and perform related duties as needed or as situations dictate and looks for opportunities to stay busy.
  25. Provide excellent customer service at all times and help to grow the customer service culture within the agency.
Qualifications

Requirements:

              Education and Experience

  • Required:  HS/GED and at least three years of experience related to Community Health, patient/client support, transportation services or related work – OR –2 years’ experience AND at least 12 months of experience as a SNAP CHW I. Must have clean driving record and a valid driver’s license. Must be able to pass Department of Transportation drug testing and secure DOT Medical Clearance. Must be age 21 or over.
  • Preferred:  Strong preference for lived or professional experience related to low-income, underserved populations, with varied health economic and educational circumstances.   Bi-lingual speaker. An AA Degree in a related field and four years of experience in fields related including Community Health Work and client transportation or some combination of education and experience totaling six years. CPR Certification preferred.

  • Must have the ability to drive in inclement weather
  • Must possess strong professional boundaries
  • Must have a strong sense of safety and adhere to safety protocols
  • Strong communication, interpersonal and customer service skills with a diverse population
  • Computer proficiency with MS Office required, with aptitude and ability to learn related technical skills
  • Ability to follow directions and work effectively in an open, fast-paced, high-stress setting with frequent interruptions
  • Good planning, organization and critical thinking skills, including ability to prioritize and communicate questions and concerns
  • Ability to work independently, using initiative and ethical judgment to resolve problems and ensure workflow is appropriately addressed in a timely manner

Training and Certifications

Securement device training within 30 days of hire

PASS training within 30 days

CPR/First Aid Certification within 60 days of hire

Trained Community Health Worker within six months of hire    

Work Environment/Physical Activities:

Work is conducted in vehicles and client homes. Work requires ability to walk clients to and from vehicle. Some lifting may be required.  May deal with difficult people, sometimes in crisis situations. This position requires local travel within Spokane County. Local travel will be approximately 50% of the position duties.
