Description

The Community Health Worker (CHW) reports to the Manager of Population Health, and must be comfortable working with individuals who may have medical and/or behavioral health conditions and/or substance use disorder. The CHW works directly with patients and their families in need of resources as well as identify and address barriers to health through screenings (i.e., gaps in care, food, housing, support, transportation).

Required Qualifications:

  • H.S. Diploma or GED
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 3 years of Bilingual fluency in one of the following languages: Spanish, Vietnamese, Arabic, Mandarin, Marshallese, etc.
  • 1 year of related Community Health or outreach experience with knowledge of Snohomish County community-based resources and services

Preferred Qualifications:

  • Associate's Degree or Bachelor's Degree in Health or Social work related field.
  • 1 year of epic appointment scheduling
  • 1 year of medicaid enrollment experience

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
