MCH Community Health Worker

Maternal Child Health (MCH)

Department of Tribal Health

Hourly Wage: $24.44/Regular/Full-Time


Provides a coordinated system of mental health support through identified stressors and needs of children, youth, and their families. The primary goal is to support children and youth (birth through 18 years) through interventions within the scope of practice through outreach and engagement, informal counseling, social support, and connection to resources addressing social determinants of mental health. Will provide services under the supervision and guidance through a team approach working with behavioral health support staff to schedule and coordinate appointments, complete follow up visits, arrange referrals to outside facilities as deemed necessary by adhering to professional standards, clinic policies and procedures, and federal, state, and local requirements. Works professionally and confidentially with parents, guardians, Tribal and IHS personnel. Follow Yakama Nation guidelines and procedures, HIPAA Federal guidelines, with best interests of the Yakama Nation and for the implementation of the Yakama Indian Nation Master Indian health P.L. 93-638 Contract and the Maternal Child Health Program.


Examples of Work Performed:

Helps patients by providing information, services, and assistance and verify patient information by interview of patient, recording medical history, and confirm purpose of visit.

Work closely with clinic staff to identify children/youth and their families in need of mental health referrals and prioritize special populations such as those who have experienced life stressors, children/youth with special needs who have experienced barriers to mental health services.

Provide tailored outreach by conducting community, home and hospital visits for newborn, prenatal and post-partum care with the focus on interpersonal and relational skills developed for families identified by care team as potentially benefiting from additional mental health support.

Provide developmentally appropriate education on identified health needs and concerns to support skill development and self-management techniques.

Advocate for and facilitate family/care-giver inclusion in decision making around the services their child is receiving or might receive. Support families when they are not satisfied with the services their child is receiving.

Bridge the gap between families and the mental health care system through application assistance, assessing behavioral health and related services, and support completing intake activities for services.

Collaborate with all staff members to provide comprehensive services that are individualized to best serve each client/family to ensure adherence to treatment and mental health recommendations.

Participate in community outreach to meet with diverse populations and build relationships and streamline mental health referrals for families.

Provide support and address barriers in obtaining health care coverage for uninsured individuals.

Support individualized goal setting with children and youth and their families to support health and mental health goals and address identified needs.

Provide culturally appropriate health education and wellness coaching on topics related to prevention, healthy living, and related health through emphasis on family and care giving strengths and utilize tools to assist with assessing need and progress.

Develop supportive, trusting relationships as means for increasing engagement in primary care and other identified services. Create the opportunity for ongoing contact and relationship building with families via phone/text messages, face-book messenger, and check-in between scheduled visits.


Knowledge, Skills and Abilities:

Knowledge of Health Services management and HIPAA.

Knowledge of Systems of Care, Historical & Intergenerational Trauma, Indian Child Welfare Act, Family-Driven Care, Laws regarding Victims of Crime, and Trauma Informed Care approach.

Knowledge of social and health services agencies of the Yakama Nation and adjacent communities as a resource to women, infants, children, and their families of the Yakama reservation.

Must have good communication skills in acting as the liaison between the patient and providers.

Must have knowledge of mental health, substance abuse treatment, social justice issues on the Yakama Reservation.

Ability to assist the mental health provider with treatment and follow up.

Ability to interact with schools, Tribal programs, community partners and governmental agencies on sensitive issues, i.e., suicide, depression, parental issues.

Ability to maintain a positive working relationship with medical staff, Tribal members, and the public.

Ability to communicate effectively both orally and in writing.

Knowledge, skills, and ability to complete required documentation of activities, objectives, and goals.

Updates professional knowledge by participating in educational opportunities.

Ability to maintain strict confidentiality in all work and program related matters.

Ability to work under stress, to organize and prioritize workload.


Minimum Requirements:

High School diploma or GED.

Minimum three years of experience in clinical setting with preferred experience in counseling/therapist office.

Must possess First Aid/CPR card.

Must have all immunizations up to date.

Must be able to work with pediatric/youth and adult patients.

Required to complete and pass a pre-employment criminal background check and subsequently obtain access to IHS EHR.

Must pass a pre-employment background check.

Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.

Required to pass a pre-employment drug test.
