Job Description:

Description: The Clinical Quality Coordinator partners with the Patient Care Team in various quality driven projects and endeavors; and act as an extension of the Patient Care Team to help achieve quality driven goals. The coordinator will be based at one of JPS's clinics. This position participates in planning, providing, and evaluating patient care at the appropriate skill level. The Clinical Quality Coordinator works under the direction of CMS Innovations and Value Based Care department and in coordination with the JPS Patient Care Team to outreach to patients whose clinical measures need to be addressed. This position partners with JPS outpatient physician offices to help achieve benchmarks for patient care and maintain/improve quality of care delivery across multiple payers and patient populations.

Typical Duties:

  1. Partners with physicians and patient care team to assist patients in completing health screens and preventive care measures that are not yet completed.

  2. Contacts external physician offices to obtain copies of consult notes and reports for patients who had preventive healthcare screens and clinical measures completed at outside facilities.

  3. Contacts Patients to remind them to pick up their Prescriptions and identifies any barriers with taking prescription or picking up prescriptions.

  4. Successfully updates preventive health screens and clinical measures in the Electronic Medical Record (EMR) that have been determined as completed.

  5. Communicates regularly with interdisciplinary team members to coordinate/update a patient's care.

  6. Maintains an accurate tracking log of patients who have been contacted and the status of their preventive health screens and clinical measures.

  7. Participates in various quality driven initiatives within JPS and collaborated together to achieve high level patient outcomes.

  8. Submits data to external agencies as directed by Sr. Clinical Quality Coordinator.

  9. Explains the importance of preventive health screens and clinical measures to staff, patients, and families.

  10. Facilitates the identification of person as high-risk or living with a disease and will work the case management team to address Social Determinants of Health (SDoH).

  11. Performs all duties at the expert level within the scope of a Medical Assistant's scope of practice. May operate diagnostic equipment (cannot interpret tests) and conduct preventative screenings, when necessary.

  12. Performs quality control checks on equipment. Prepares and sterilizes medical equipment. Fosters accountability for clinical practice and patient goals.

  13. Records patient care documentation in the EMR accurately and in a timely manner. Identifies when others' documentation is incorrect and alerts clinic leadership. Assists other MA's with documentation questions.

  14. Coordinates patient care as directed by provider, and policies/procedures.

  15. Respects patient confidentiality at all times. Ensures others are also respecting patient confidentiality.

  16. Anticipates provider's needs prior to patient encounter.

  17. Partners with multidisciplinary care team to support and improve med adherence.

  18. Performs telephone and in-person screening limited to intake and gathering of information without requiring the exercise of judgment based on clinical knowledge.

  19. Performs patient outreach and engagement to ensure patient awareness of clinic appointments and adherence to their person centered plan.

 

Qualifications:


    Required Education and Experience:
  • High School Diploma, GED or equivalent.
  • 1 year of experience in the healthcare field.

  • Required Licensure/Certification/Specialized Training:
  • Current certification from a national registry as a Certified Medical Assistant (CMA).
  • OR
  • Current licensure to practice vocational nursing in the State of Texas by the Board of Nurse Examiners and must maintain licensure throughout employment.OR
  • Requisite educational or training background and 1 year of combined experience in medical billing and coding, patient registration, and navigating patient's electronic medical records may suffice for licensure/certification.

  • Preferred Education and Experience:
  • Associate's Degree from an accredited school of professional nursing.
  • OR
  • Successful completion of a Medical Assistant training, coding and billing training program or graduate from an accredited School of Vocational Nursing.

  • Preferred Licensure/Certification/Specialized Training:
  • Current licensure to practice vocational nursing in the State of Texas by the Board of Nurse Examiners and must maintain licensure throughout employment.
