Position Summary:
The Clinical Care Coordinator is a professional member of the health care team who assumes advocacy and accountability and works in collaboration and continuous partnership with chronically ill or high-risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach to provide a continuity of the single standard of care.
Duties & Responsibilities:
- Independently coordinates and tracks patient care, including discharge planning for patients.
- Provides discharge and educational instructions for patients and their families.
- Collaborates with resources to implement educational programs.
- Participates in developing inpatient care plans specific to patient age and developmental needs.
- Utilizes clinical pathways in supporting care provision with the multidisciplinary healthcare team.
- Collaborates with healthcare team members and external payer sources for care delivery from hospitalization to discharge.
- Tracks quality measures related to care coordination efforts.
- Screens patient records to identify adverse events or sub-optimal care patterns using specific criteria and judgment.
- Reviews and interprets clinical data to identify care variations and makes recommendations for improvement.
- Analyzes collected data, evaluates current practices, and monitors issue resolution status.
- Coordinates physician peer review committees and presents information to appropriate committees.
- Collaborates with medical staff and Risk Management for quality patient care and resource utilization.
- Works closely with JMH Clinical Performance Specialist for updating nursing standards and policies.
- Facilitates completion and updates of policies and procedures.
- Ensures integration of policies and procedures into the current system and archives outdated documents.
- Conducts monthly unit educators' meetings and updates hospital-wide manuals.
- Participates in hospital-wide quality improvement activities and JCAHO survey preparation.
- Conducts record review audits and unit inspections for JCAHO compliance.
- Participates in community seminars and continuing education programs for knowledge enhancement.
- Assess complex patient care needs and coordinates patient care using specific criteria and evidence-based guidelines.
- Completes electronic documentation for case management activities and communicates effectively with stakeholders.
- Articulates relevant information at meetings, rounds, and to administration and medical advisors.
- Demonstrates effective interpersonal communication skills with healthcare teams and administrative staff
Experience:
Generally requires 3 to 5 years of related experience. Registrar experience highly preferred.
Education:
BSN or Bachelor's degree in related field is required. Master's degree is preferred.
License Certification:
Valid Florida RN license is required. American Heart Association Basic Life Support (BLS) and any additional applicable life support certification for Healthcare Providers is required upon hire with at least 6 months validity and maintenance at JHS for the duration of employment. Must meet and maintain valid and current all unit specific and organizational skills/competencies, certifications/licensures, as required by regulatory and/or nursing standard of practice for the specialty.
