Tallahassee Primary Care Associates is looking for a full-time LPN Clinical Care Coordinator.
The qualified candidate must be able to work Monday-Friday, 8 a.m. - 5 p.m
I. Minimum Qualifications
- Graduate of an accredited school of nursing
- Current Healthcare Provider Training (BLS)
- Active Florida LPN license in good standing
- Knowledge of medical terminology
- Minimum 2 years previous clinical experience
- Knowledge/background in clinical quality measures
- At least 2 years experience in care management is preferred
- Proficiency with Electronic Medical Records (EMR) and Microsoft Office Products (Excel, Outlook)
- Ability to work well with others and under pressure
- Ability to work individually
- Ability to communicate with patients and physicians face to face and via telephone
- Ability to evaluate and communicate HEDIS and other quality measures
- Strong attention to detail and organizational skills
- Strong customer service skills
II. Essential Functions of Position
A. Administrative Clinical Responsibilities
- Medical chart review/audit to ensure compliance and pre-set quality improvement program guidelines to close Quality Care Gaps.
- Direct and participate in the development of patient care and quality driven protocols and policies.
- Input Health maintenance data into EMR to reflect chart audit results.
- Utilize EMR and payer dashboards to effectively manage Quality incentive programs like CHP QPP and CMS Star quality measures and initiatives.
- Provide EHR support to providers and their offices. Back-up to Quality and Improvement EHR Support Coordinator
- Following protocol approved by Exec. Comm., input orders for labs, diagnostic tests, input referrals for health maintenance and input refills to 90-100 days as appropriate.
- Perform patient outreach via phone, letter or web messages to assist patients with becoming healthier by following the healthcare provider's recommendations/orders.
- Work closely with providers, office managers, and staff regarding patient health and closing care gaps. Work with the pharmacies on medication adherence and/or payers on quality measures.
- Serve as the contact-point, advocate, and informational resource for patient, family, care team, payers, and community resources.
- Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
- Serve as linkage between primary care physicians and patients' health providers to improve coordination of services and information flow
- Facilitate and attend meetings between care team, payers, and community resources, as needed.
- As needed, perform patient care duties within the scope of an LPN license.
- Complete 1 hour of Self Training per month and cross training within the team. Provide training to providers and their offices regarding new QPP or HEDIS measure guidelines.
B. Communication
- Communicate with TPCA offices to review the clinical guidelines implementation process
- Communicate with patients and/or their families to effectively create care plans and goals
- Communicate with physicians, social workers and community resources to coordinate patient care services
