Job Description
The Clinical Care Coordinator is responsible for managing communication between patients, family members, and care providers. They manage patient care plans, keep patient schedules up to date, and act as a liaison between the patient and clinician by keeping the lines of communication flowing.
- Schedule patient visits and answer pre-visit questions including billing and preparation.
- Manage and Coordinate an assigned number of care plans.
- Educate patients on care plans, discuss cost of service, and payment options
- Follow up on patient intakes including collecting required information and sending and reviewing forms
- Keep clinician calendars up to date
- Provide information on all NBI services
- Create progress notes for Clinicians
- Manage patient accounts including contact information, financial information and medical records in a confidential manner.
- Answer patient calls, and emails and resolve all inquiries requests
- Greet and check in patients
- Check emails daily for patient cancelations / updates and Clinician updates
- Provide community resources and referrals for continuing care
- Keep Office supplies inventory
- Answer phone calls/ Redirect phone calls to appropriate person
- Performs other clerical or admin duties as assigned.

Required Experience and Education:
- High School degree required
- Associates Degree or Undergraduate Degree Preferred
- Previous customer service experience preferred
- Previous experience in a clinical setting preferred
- Experience with Microsoft office and TEAMS a must
Required Knowledge, Skills and Abilities
- Excellent verbal/written communication skills coupled with strong interpersonal skill. (bilingual in Spanish preferred).
- Ability to promote and maintain a flexible, positive, team-oriented, and customer-focused attitude within and between departments.
- Ability to deal professionally, courteously, and efficiently with the patients and staff.
- Must demonstrate patience, empathy, and respect.

- Knowledge of clinical terminology, spelling and office ethics
- Excellent administrative, data entry, and customer service skills.
- Must be able to multi task, learn quickly and adapt to change easily.
- Excellent prioritization skills.
- High level of professionalism and demonstrated comfort level in speaking directly to external and internal customers/vendors
- Must be able to handle sensitive patient information and honor the confidentiality of all patient records.
