Salary: $20 - $22 / hour
Hours: Sunday- Thursday / 8:00 AM - 5:00 PM

Qualifications
Be the Best Continuous Card Coordinator You Can Be
If you meet these qualifications, we want to meet you!
- A minimum of one year of hospice experience preferred
- A minimum of one year of supervisory, Continuous Care coordination, scheduling or triage experience
- Demonstrated leadership and organizational skills
Required Certifications and Licensures:
- Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) or Registered Nurse currently licensed in the state(s) practicing preferred. Can be a non-nurse if the administrative team agrees on the candidates' qualifications
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver who can travel to all business locations
