JOB TITLE: CASE MANAGER
DESCRIPTION:
The primary purpose of your job as a Case Worker/Administrative Assistant, is to provide participants with assistance in Entitlements, schedule doctor/transportation appointments, pay bills, recertify Medicaid, send faxes for participants on an individual basis. Additionally, he/she will be involved with assisting the Program Director with any other needs that the Center staff will have.
RESPONSIBILITIES:
Under the supervision of the Program Director the responsibilities will include but not be limited to the following:
ü Maintain and update member records
ü Case management
ü Entitlements
ü Assist members with paying bills, scheduling doctor appointments and transportation to and from appointments
ü Must have knowledge of Medicaid rules, SSI, SSA, SSD
ü Fulfill miscellaneous duties as necessary including helping out members with concerns and questions
ü Submit program reports, as necessary; and
ü Providing or arranging services for participants as needed
ü Write intakes/Assessments/Care Plans as needed
ü Coordinating services for participants
