The incumbent serves as the COMPACT Act Coordinator for NY Harbor VA Healthcare System. The Social Work Program Coordinator serves in facilitating implementation of COMPACT Act, which includes education of providers, internal stakeholder, external stakeholders, Veterans, and their families related to COMPACT Act.

Duties
The incumbent Social Work Program Manager will dedicate 65% of their time to Care Coordination and Integrated Case Manager (CC&ICM) and 35% of their time to Compact Act Coordinator (CAC) duties.
Compact Act Coordinator:
The incumbent implements COMPACT Act, which includes education of providers, internal and external stakeholders, Veterans, and their families. The incumbent is point of contact and facility subject matter expert on COMPACT Act, and provides clinical case management to Veterans utilizing COMPACT Act services. Assumes responsibility for evaluating implementation efforts at the facility and VISN, if requested, and ensures adherence to policies and procedures from the Office of Mental Health and Suicide Prevention. The incumbent uses his/her educational training, clinical experience, VA policies, and Joint Commission standards to administer direct services to a Veteran population.
The incumbent performs the following duties:
- Provides therapeutic services to individuals, families, and groups covering a wide variety of psychosocial issues not limited to the mental health setting.
- Collaborates with interdisciplinary teams in the provision of comprehensive health care services to Veterans and ensures equity of access, service, and benefits to this population.
- Collects, develops, and analyzes data using appropriate clinical indicators to monitor and assure the care provided is of the highest quality; provides consultation and education to VA staff and community-based service providers.
- Documents clinical activities and interventions in computerized patient record system (CPRS) and ensures appropriate transfer of Veteran care
- Provides clinical psychosocial treatment and case management services at an advanced practice level to eligible Veterans and their family members. Veterans may come from a wide geographic area, with complex medical and mental health problems that require intense clinical oversight and creative problem solving.
- Provides community-based case management services to Veterans with complex mental health and psychosocial needs, severe functional impairment, and high inpatient and emergency department utilization, in coordination with existing community and VA services. Services provided are recovery focused based on Veteran's self-identified goals, strengths, and values to improve their physical and social environments. Services may demand a high amount of drive time as interventions occur in the community and multiple visits per week may be made to the Veteran as needed.
Care Coordinator and Integrated Case Manager (CC&ICM):
The CC&ICM Social Worker demonstrates advanced practice skills and judgment. CC&ICM is a specialized framework that provides structure and standards for multiple healthcare professions throughout the continuum of care to collaborate and provide optimal utilization of healthcare resources. It uses a biopsychosocial model to enhance Veterans' wellness, level of functioning, and quality of life. The CC&ICM model goals include, but are not limited to, bridging medical and mental health care, transitioning inpatient and outpatient care, and integrating community care and VA Care thereby creating a seamless continuum of experience for the Veteran. This provides safe, efficient, and cost-effective interventions integrated throughout the continuum of healthcare to improve access, reduce duplication and fragmentation of services, ensure adherence to standards of practice, improve patient satisfaction, and support staff.
- The incumbent is responsible for case identification through evaluation and data analysis, direct case management services, facilitation of the Care Coordination Review Committee (CCRT), and direct collaboration with nursing and social work CC&ICM leads. The health care and psychosocial needs of the population served are complex and require a high degree of independence, clinical oversight, and creative problem solving.
- The incumbent builds and fosters networks for referral and Care Coordination, provide support and education to staff throughout the medical center, and work collaborative within multiple care settings to meet the needs of those served.
- The incumbent interfaces with all Integrated Care Communities (ICC) across the health care system. The duties and responsibilities of this position require and authorize the individual to develop and implement the local policies of the program.
- The Incumbent is administratively responsible and accountable for the clinical and program development and effectiveness of the facility CC&ICM Program.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure: Persons hired or reassigned to Social Worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website.
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.
Grade Determinations: GS-12
Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as Social Worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits).
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs(a document with written demonstration of KSAs must be included with application, see required documents section).
Knowledge of program Coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
