Founded in 1977, SCAN-Harbor is a non-profit youth and family service organization that provides a variety of integrated supports to the highest risk children and families of Harlem, East Harlem and the South Bronx. Our mission is to provide the highest risk families and children living in Harlem, East Harlem, and the South Bronx with integrated family-focused programming that uses a positive approach, harnesses individual strengths and fosters responsibility, self-esteem, initiative and the development of life skills. SCAN-Harbor has been led by its Executive Director, Lewis Zuchman, an original 1961 Freedom Rider, since 1987. The agency currently has over 100 full-time and 300 part-time employees with an annual budget of approximately $23 million.

Job Description:

We are seeking a dynamic and experienced Case Planner Supervisor to lead our team of dedicated professionals. As a Case Planner Supervisor, you will oversee and manage a team of Case Planners, ensuring the provision of high-quality services to our clients. Your role will involve supervisory responsibilities, case oversight, staff development, and maintaining adherence to agency standards and procedures

Responsibilities:

• Supervise, mentor, and support a team of Case Planners.
• Provide guidance and leadership in the development and implementation of case plans.
• Review and approve case documentation, ensuring accuracy and compliance.
• Conduct regular case reviews and provide feedback to staff.
• Collaborate with other departments and agencies for holistic client support.
• Ensure that all services provided meet regulatory and agency standards.
• Participate in the recruitment and training of new staff members.
• Handle escalated client issues and concerns as needed

Requirements:

• Bachelor's or Master's degree in Social Work or a related field.
• Proven experience as a Social Worker or Case Planner, preferably in a social services setting.
• Fluency In Spanish
• Strong knowledge of social work principles, theories, and practices.
• Excellent assessment and counseling skills.
• Ability to develop and implement effective care plans based on client needs.
• Knowledge of community resources and the ability to connect clients with appropriate services.
• Empathy, compassion, and the ability to build rapport with diverse populations.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work collaboratively in a team environment.
• Proficient computer skills for documentation and case management purposes.
• Valid driver's license and reliable transportation (if required).
• Provide case management and advocacy services. In addition, experience working with children and families
• Strong organizational skills, high attention to details, effective communication skills, computer proficiency and knowledge of ACS Prevention Services and case planning
• Connections and PROMIS are a plus
