United Jewish Council of the East Side (UJCES), a CBO on the Lower East Side of Manhattan is looking for a dynamic person to join our team of professionals as the Case Manager for our older adult center.
Education:
- BA/BSW in Human Services or other relevant field preferred
OR
- HS Diploma with equivalent years of experience in Human Services, preferably with older adults.
Responsibilities:
- Consistent research of local and citywide resources for older persons
- Provide Case Assistance to walk-in clients or to those who call for assistance, accurately inputting that data and case notes
- Provide Information and Referral to walk-in clients or to those who call for assistance
- Follow up in a timely manner on referrals and case assistance
- Work with clients in a manner that is empathetic, respectful and patient
- Accompany clients as needed to city benefit offices such as public assistance, social security, Medicaid and HRA to advocate on their behalf at fair hearings and for application process
- Pre-screen clients and apply on their behalf for Medicaid, EBT and other benefits
- Make engaging daily birthday calls to clients and updating their annual contact and other information
- Conduct annual Nutrition Screening Initiative (NSI) identifying clients that need nutritionist and then providing those referrals
- Welcome and sign clients in for daily lunch and collect daily contributions in a welcoming and engaging manner
- Promote and encourage client participation in activities such as lunch, classes and special events via phone-calls, flyers and social media
- In conjunction with the Home Delivered Meals Coordinator, respond to HDML deliverers’ “No Answers,” and check in on those clients to ensure their safety
- Supervise clients on the center’s trips and outings
- Ensure all current and new clients of the center fill out and sign NYC Aging’s Consent to Collect Data forms and all Case Assistance clients fill out NYC Aging’s Client Consent forms
- Provide other human service and administrative duties with a can-do attitude, as needed and identified by the Director, Assistant Director and Social Work Supervisor.
Salary:
$45,000-$50,000 a year plus fringes benefits.
