The Case Supervisor staffs new and existing clients, supervises the delivery of care to participants, and addresses concerns brought up by both their field workers and participants. The Case Supervisor also maintains records for both field workers and participants and may be called to engage in community awareness activities outside the office.

  • Provide new worker requirements to Applicant Coordinator/Central Compliance
  • Staff new clients
  • Re-staff existing clients as needed
  • Handle daily scheduling
  • Handle non-compliant timesheet workers
  • Meet with field workers as needed on payroll day
  • Update incident reports as needed
  • Perform disciplinary action for field employees as needed
  • Report Critical Incidents to Lead Supervisor
  • Record and report to Case Managers and in the client notes, all deviations from the Plan of Care sent by the CCU/MCO
  • Community Outreach

  • Audit Files for Transfer to another HCP Branch
  • Ensure compliance with Client Folders
  • Ensures timely Client Visitation
• Ensures compliance of timely performance evaluations for field workers under their supervision.
• Ensures compliance of timely quarterly conferences for field workers under their supervision.
• Maintains company standards of continuity of care by working with the Person-Centered Planning Team.
• Works with other branch staff such as Compliance and Applicant Coordinators, Field Support Personnel, Training and Payroll Staff to support smooth branch operation
• Works with the HR Department to handle unemployment and workers compensation claims of their field workers.
• Document changes in participant health and living status in the Portal.
• Monitor FHCA services by conducting monthly phone calls to participants.
• Other activities as determined by their supervisor as needed.
