LSSI is hiring a full-time Manager of Transition Case Manager. Supervise and coordinate case management services in the Association House Transition Team, provide clinical supervision to staff, conduct weekly staff meetings, complete necessary compliance and monitoring activities and reports, and ensure that Williams and Colbert Class Members are transitioning into the community in a safe and expedient manner in compliance with DHS terms.

Essential Functions:
- Provide direct supervision to assigned staff.
- Conduct staff meetings and coordinate weekly schedule with RN/OT/SOAR/etc.
- Ensure all staff documentation is completed in a timely manner and in compliance with Rule 140 standards. Ensure all client charts comply with Rule 140 standards.
- Ensure compliance with DHS and Williams and Colbert Consent Decree standards.
- Train new staff to ensure proficiency in service delivery, comprehension of and compliance with Rule 140 and accreditation and licensure standards, and proficiency with Electronic Health Record.
Position Qualifications:
- Master's degree in social service field required.
- Must have minimum of two years' experience in social services with increasing responsibility over time.
- Experience supervising staff is preferred.
- Excellent written and verbal communication skills.
- Drive to multiple facilities across the Chicagoland area and surrounding suburbs. Valid Illinois Driver's License and Insurance required.
