Title:Case Manager Supervisor
Location: 2850 S. Wabash
Shift: Monday- Friday (8:30am to 5:00pm)
Summary: Under the general guidance of the Program Manager, the Case Manager Supervisor provides a broad range of case management services within guidelines established by the Organization and SEED program. The Supporting Education and Employment Development (SEED) is a thirteen-month pre-plea deferred prosecution program that aims to serve emerging adults charged with delivery of a controlled substance engage in education, job training, and related supports. It seeks to improve lives through meaningful participation in education, social services, and career development, to decrease future involvement with the criminal justice system. Individuals with criminal records and other lived experiences are encouraged to apply.
The Case Manager Supervisor leverages industry knowledge and best practices to promote the mission and vision of Heartland Human Care Services. Supports culture development and management efforts.
Essential Functions:
Duties/Responsibilities

  • Collaborate with SEED Program Manager and Career Pathway Supervisor to facilitate participant case staffing
  • Establish and maintain collaborative working relationships with staff across Heartland Human Care Services
  • Manage Heartland Human Care Services involvement in court calls, including collecting and reviewing progress reports from staff, and submitting them to the court on time, and answering questions during court calls
  • Provide input on performance tools and establish routines for collecting, reviewing, and utilizing data to understand and assess participants’ behavioral change and to ensure accountability
  • Manage all necessary paperwork and reporting associated with orientation, case management activities, court contacts, participant advancement, retention follow-up, and other programming activities
  • Conduct Monthly Supervision with Job Coaches
  • Maintain regular communications with SEED Program Manager that promptly address any obstacles faced with service provision to participants, staff, and partner organizations, and oversees quality improvement
  • Contribute to organizational culture aligned with core commitments of tackling root causes; encouraging creativity and innovation; using data for continuous improvement and to measure progress; applying an equity lens to all that we do; partnering with participants and others to co-create and achieve ambitious, shared goals; and aligning and capitalizing on our global and domestic efforts and perspectives
  • Refers program participants to community resources and other organizations, determines program participants’ eligibility for financial assistance, and provides financial assistance and support to program participants in accordance with Organizational policies and procedures.
  • Perform all duties of a Case Manager when necessary
  • Completes accurately and in a timely manner all necessary forms, case records and statistical reports. Submits such documentation to the supervisor/manager within the designated timelines.
  • Transportation of program participants and/or Organizational property may be required.
  • Other duties may be assigned.

Education and Experience:

  • Bachelor's degree in Social Work, Psychology or related discipline required master's degree preferred
  • Minimum five years’ experience in casework, human services or related field is required.

Certifications/Licenses:

  • A valid driver’s license may be required.

Knowledge/Skills/Abilities (K/S/A)

  • Excellent customer service and organizational skills.
  • Knowledge of the local ecosystem of support services providers.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.
  • Ability to draft routine reports and correspondence.
  • Ability to prioritize work in a high-volume environment.
  • Commitment to the mission and values of HHCS, as demonstrated by a professional approach that is strength-based, trauma-informed, inclusive, and grounded in human rights
  • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders both internally and externally.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.

Work Arrangement:
This role is eligible for a “hybrid” work arrangement. It is expected this person would be able to meet for “in person” meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.
