Job Scope: This position provides a mixture of direct-service to clients and supervision duties to full-time staff by supervising a team providing assessment, short-term focused Case Management, and linkage to housing and alternatives to long-term care to adults with serious mental illness. Treatment services are delivered at flexible times that meet clients’ needs and in settings that offer the best potential for positive outcomes, including in hospital settings, in-home, and in community-based settings. Provide strengths-based, client-centered, and trauma-informed assessments and services to adults with severe mental illness, history of homelessness, and co-occurring substance use disorders. Provide program orientation, monitor staff documentation, complete payroll and co-lead team meetings. Primary liaison with external providers and short-term housing partnerships.
Responsibilities
- Develop a strong range of immediate and temporary housing opportunities, including hotels, crisis beds, and KYC apartments
- Conduct phone check-ins and/or in person meetings to ensure client contact
- Ensure coverage across service areas; and oversee the day-to-day operations of clients enrolled in Front Door program
- Communicate team expectations regarding linkage, coordination, and level of care needs; will ensure that employees are well-informed about alternative services in the community in order to collaborate with partners to meet the needs of clients
- Ensure completion of authorization, documentation, and charting in compliance with grant requirements, state specific accreditation, and agency standards.
- Communicate with Manager regarding client caseload; and collaborate with team to provide consultation and information sharing
- Provide immediate crisis intervention services or on-call coverage as needed
- Master’s Degree from an accredited college or university in social work, psychology, or counseling.
- Highly preferred LCPC or LCSW, will consider within 6 months of hire
- 2-3 years of experience working with adults with serious mental illness
- Preferred 1-2 years supervisory experience
- Preferred experience working with homeless populations and/or co-occurring substance use
- 1 year experience working in non-traditional treatment environments i.e.in-home, residential, hospital, community settings
- IM-CANS certified or obtain within 30 days
- Applicant must have a valid IL Driver’s License and daily access to a well maintained/insured vehicle
- Basic computer and smartphone proficiency, knowledge of Word, Excel, and internet skills
- A majority of the employee’s time will be spent in the community, driving to and from and providing services in community settings, public entitlement offices, short-term housing settings, and client homes
- In supporting clients to live independently, employee may be exposed to varying home conditions
- Will be required to drive daily for meetings, assessments, and client services in and around Chicagoland area, as well as meetings/trainings weekly at the Mt. Prospect, Elk Grove, Rolling Meadows, and Arlington Heights locations. Coverage and Crisis services may be required in greater FDDP service area with includes Lake, Dupage, and Kane Counties.
- Exempt, Full-Time, 37.5 hour work week
- Must work a flexible schedule in response to the needs of the target population served. Work hours are weekdays with rotating weekend availability. Ability to choose defined weekend shift if desired.
