QIDP Responsibilities
- Relate with case Management companies and serves as Anointed Home Liaison
- Develops for each consumer, an individual support plan (ISP) and monitors its implementation. Revises ISP’s as needed to ensure that consumers’ health, safety and programming needs are met in a timely and effective fashion.
- Implement programming revisions as indicated per the ISP’s recommendations.
- In waiver settings works cooperatively with the case manager through the person centered planning process.
- In group home settings, functions as the facilitator for each individual’s person centered planning process.
- Conducts programming reviews as outlined in the consumer’s ISP and in accordance with company policies and procedures to include review, analysis and electronic filing of program documentation.
- Communicating significant observations and recommendations to the ISP as appropriate.
- Routine interviews with individuals on case load to include checks for any abuse, neglect or exploitation.
- Maintain regular contact with outside vendors and families.
- Establishes and maintains a communication system with day program providers to assure that individuals vocational needs are met, and service conditions are up to standards.
- Any grievances are addressed in a timely manner and reported to the Program Director.
- Timely reporting and follow up of incidents requiring BDDS incident reports.
- Participate in area on call rotation as assigned by the Program Director to address critical consumer and staffing issues are handled in a timely and effective fashion.
- When no other staffing options are available, provides direct care for program sites.
- Act as the lead member of the IST, attends meetings at least semi- annually and on an as needed basis.
- Complete reviews of goals preformed in accordance with the ISP and enter into the electronic chart for all assigned homes.
- Create new ISP within first 30 days for all new admissions to group homes sites.
- Enter all goals into electronic system within 30 days for all new admissions.
- Management of consumer electronic charts and oversight for compliance
- Professional presence in assigned homes
- Management of Survey process for all assigned homes and covering when needed for other homes.
- Create Plans of Correction in conjunction with QA and implement in the homes and monitor for compliance
- Sit on the Human Rights Committee (HRC) as assigned, prepare HRC related items as approvals are needed.
- Completion of all assigned investigations within guidelines.
- Assist with the admission and discharge process for individuals
QUALIFIACTIONS:
Required Qualifications:
- Active and unrestricted Social worker license in the state of Indiana or Master’s level social worker
- Experience with BDDS is a MUST
- Experience in providing care coordination to clients with disabilities
- Experience facilitating and working in coordination with an Interdisciplinary Team
- Experience coaching or mentoring staff
- Intermediate level of experience with Microsoft Word and Excel
- Proven ability to navigate a TEAMs environment
- Resident of Indiana
- Able to work from the office. Not a remote job.
Preferred Qualifications:
- Case Management experience
- Experience with LTSS or Waivers
- Background in managed care
