POSITION SUMMARY:

A Case Manager is a person who delivers services under the direct supervision of a QMHP (Qualified Mental Health Professional). A QMHA/Case Manager provides services and support for clients that help them develop appropriate skills to increase or maintain their level of functioning. Services may include the coordination of mental health services, rehabilitation services, social services, personal services, medical, dental and other health care services, educational services, financial management, legal services, vocational services, transportation, recreational and leisure activities, and other services required to meet their needs as identified in their personal care plan.


MINIMUM QUALIFICATIONS:


  1. Education: Bachelor's degree in a behavioral sciences field; or a combination of at least three years relevant work, education training or experience; and able to demonstrate the competency necessary to fulfill the position.
  2. Licensure/Certification: Current class C Oregon Driver's License Required. Must maintain a clean driving record. Must pass a criminal histrory check as required by ORS 443.004. Must be eligible for certification as a Qualified Mental Health Associate (QMHA) through the Mental Health and Addiction Certification Board of Oregon (MHACBO). Have or obtain CPR/BLS certification.
  3. Experience: Three years combination of training or experience in psychology, counseling, or a related field. May have college coursework in psychology, social work or related social sciences, experience in a social service setting; or any satisfactory equivalent combination of experience (professional and/or personal) and training which demonstrates the ability to perform the position duties. Cerner or other eHR systems experience required. Microsoft Office Suite experience required.
  4. Job Knowledge: Demonstrate the competency necessary to communicate effectively, understand mental health assessment, treatment and service terminology and apply these concepts. Provide psychosocial skills development; implement interventions as assigned on an individualized service plan (ISSP) and provide behavior management and case management duties.
  5. Skills: Must have the skills necessary for interacting with the public and the ability to coordinate services across various agencies and community groups. Must have the ability to communicate effectively. Must have an ability to prepare concise and complete case notes. Able to be flexible and work effectively with youth, families, adults, and collaborate with community agencies. Must be computer literate. Must understand HIPAA regulations. Must have the ability to demonstrate effective time management skills while working independently with minimal supervision.
  6. Expected Hours of Work: Typical hours of operation are M-F, 8:00am-5:00pm. Must be able to work a flexible schedule, as needed.
  7. Personal Protective Must be able and willing to wear appropriate Personal Protective Equipment (PPE): Equipment (PPE) required to perform the job safely.
  8. Supervisory Responsibility: This position does not directly supervise staff.
  9. Travel: Travel (using a District vehicle) occasionally required for this position. LDH driver training required.

ESSENTIAL FUNCTIONS

  1. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.
  2. Works with the care team to assist the client in meeting their treatment goals and access natural resources and supports within the community.
  3. Work with hospitals and other residential institutions to facilitate discharge planning and transition to community placement. Develop effective working relations and cooperate with medical team throughout the entire case management process.
  4. Assists QMHP's in facilitation of group skill training that teach basic living skills and promote social reintegration.
  5. Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  6. Serve as a liaison between the client and social, financial, health, and legal services.
  7. Work collaboratively with other community agencies to coordinate treatment efforts and share pertinent information.
  8. Provides timely and accurate documentation as required per OAR's and Lake District Wellness Center policies and procedures.
  9. Report any suspected violations of client's rights or abuse of a client by another client or a staff member according to policies and OAR's regarding mandatory reporting. Such reporting is required for, but is not limited to, suspected incidents of physical assault, neglect of care, sexual or financial exploitation, which may include predatory individuals who borrow money or removing property from the home of the client.
  10. Attends staff meetings and training sessions as scheduled.
  11. Admit new clients by reviewing records and assisting with the completion of the client intake process. Interview clients to assess their situational needs.
  12. Assist the QMHP in tracking progress towards goals given on client's Treatment Plan, as well as assisting the clients to help them work towards these goals.
  13. Works one on one with clients or in groups to help them develop basic life skills.
  14. Follows the policies and procedures of Lake District Wellness Center.
  15. Obtain and maintain AVADE training level as determined by supervisor.
  16. Other duties as assigned: the essential functions outline the primary duties performed by this position, however, other related work assignments may be added, removed, or changed by management.
