We need compassionate RNs and LPNs to join our team of Case Managers. We serve vulnerable children and families in Marion & surrounding counties. Our Nursing Case Managers provide education and skills training to families and children to ensure compliance with healthcare needs. The objective is to safely reunify clients with family. Ideal candidates have experience working with children, teens, and adults. Includes a moderate amount of local travel.

LPNs earn $35 - $40 per hour to start! RNs earn $45 - $50 per hour to start.
Full-time caseloads available.
Duties include:
- Developing detailed Nursing Plan of Care based on in-depth assessment of living environment
- Education of family and youth on proper management of diseases and medical conditions
- Coordination with members of the multi-disciplinary team to meet client's holistic needs
- Oversight of the complex medical needs of clients
Requirements:
- Must be a Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) holding an unencumbered license in the state of Indiana
- Must have a minimum one (1) year of long-term care experience as a licensed nurse
- Ability to pass our comprehensive background and drug screening process
- Requires a moderate amount of travel to client sites
Other Requirements:
- Current CPR certification
- Current physical exam
- Current 2-step PPD
- Valid driver's license and auto insurance
