Description
The Housing Stability Grants area of Health and Human Services department is seeking two Case Management Assistants to support the school to housing program. These positions will receive referrals from public school districts for families that are identified as, at risk of homelessness or unstably doubled up. These positions will support families through assessing their housing needs and identifying resources within the County or community to meet those needs, while working in close coordination with school partners and contracted housing providers.
Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500referral bonus. For more information visit the employee referral program page.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:
- Respond to families at-risk of losing their housing or doubled up referred by public school districts.
- Conduct trauma-informed, strengths-based assessments.
- Identify resources to support stable housing by determining client's eligibility for various rental assistance programs.
- Complete applications for rent assistance and enter data into the RentHelp Housing and Development Software (HDS) database.
- Attend weekly case consultation with school districts and service provider partners.
- Communicate in a professional and timely manner with families, schools, and housing partners.
- Maintain a caseload of 25-40 families for light touch or short-term services, including initial assessments.
- Serve as team lead for AmeriCorp members, volunteers, and interns by providing support for onboarding tasks.
- Attend all one to one, unit, and program meetings.
Need to have:
- One of the following:
- Two years or more of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two years or more of college coursework in the behavioral sciences.
- Experience:
- Using databases and/or computer programs such as HDIS or HDS to perform data entry where accuracy is essential.
- Working with families and/or individuals at risk of losing housing or experiencing homelessness.
- Accessing community resources and/or developing partnerships with other agencies such as schools or external organizations.
- Using Microsoft Office Suite (Excel, Outlook, Teams, and/or Word).
- Knowledge of community resources and social services available to clients.
- Ability to:
- Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
- Make sound, independent judgment.
- Work effectively and collaboratively with external and internal partners.
