Weekends: rotating manager on duty

Compensation: $80,000-$90,000
Overview:
The Assistant Director ofSkilled Nursing with Case Management Experience plays a crucial role inoverseeing and managing the Skilled Nursing services within a healthcarefacility. This position requires a combination of clinical expertise,leadership skills, and proficiency in case management to ensure the higheststandards of patient care and efficient utilization of resources.
Responsibilities:
1. Clinical Oversight:
- Supervise and coordinate Skilled Nursingservices to ensure the delivery of high-quality patient care.
- Collaborate with healthcare professionals,including nurses, therapists, and physicians, to develop and implementcomprehensive care plans.
- Monitor and evaluate patient outcomes,ensuring adherence to established clinical protocols and standards.
2. Case Management:
- Utilize case management principles toassess, plan, implement, coordinate, monitor, and evaluate the options andservices to meet the individual's health needs.
- Work closely with interdisciplinary teamsto facilitate seamless transitions of care and efficient utilization ofresources.
- Conduct regular reviews of patient cases,identifying opportunities for improvement in care coordination and resourceutilization.
3. Leadership andSupervision:
- Provide leadership and direction tonursing staff, ensuring a positive and collaborative work environment.
- Foster a culture of continuousimprovement, encouraging staff development and ongoing education.
- Collaborate with other departments tooptimize overall healthcare delivery and outcomes.
4. Compliance and QualityAssurance:
- Ensure compliance with regulatorystandards, policies, and procedures related to Skilled Nursing services.
- Participate in quality assuranceactivities, including audits and performance improvement initiatives.
- Implement strategies to address andresolve any identified compliance or quality issues.
5. Communication andCollaboration:
- Facilitate effective communication betweenhealthcare professionals, patients, and their families.
- Collaborate with other departments, suchas social services, rehabilitation, and administration, to achievecomprehensive patient care.
6. Resource Management:
- Manage and allocate resources efficiently,including staffing, equipment, and supplies.
- Identify opportunities for costcontainment without compromising the quality of care.
Qualifications:
- Registered Nurse (RN) witha current and active license.
- Bachelor's degree innursing; Master's degree in healthcare administration or related fieldpreferred.
- Proven experience inSkilled Nursing and case management within a healthcare setting.
- Strong leadership andmanagement skills.
- Knowledge of regulatoryrequirements and standards related to Skilled Nursing.
- Excellent communication,interpersonal, and problem-solving skills.
