The Women's and Children's CARE Coordinator is the designated coordinator for the Connect, Assist, Recovery, Encourage (CARE) Project at WVU Medicine. The CARE Project is a comprehensive and integrative medical and behavioral health program for pregnant and postpartum women. The project supports healthy baby outcomes by providing prevention, early intervention, addiction treatment and recovery support services. The primary role of this position is to serve as an advocate and navigator for the mother by engaging with service or referral agencies including, but not limited to, medical providers, hospital staff, mental health providers, and community, state, or federal resources.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor of Science in Nursing (BSN) AND WV RN Licensure OR Bachelor of Social Work (BSW) AND LGSW/LCSW/LICSW certification in the state of West Virginia.
2. Must possess valid WV or neighboring state driver's license and must maintain WV or neighboring state minimum auto insurance.
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Addictions counseling certification preferred.
EXPERIENCE:
1. Experience in maternal and/or child health, behavioral health, substance abuse, counseling, and/or social services experience in care coordinator using appropriate internal and external resources preferred.

1. Maintains knowledge of current treatment and management of neonatal abstinence, evidence based addiction programs and children's family services, and internal/external community resources.
2. Maintains effective communication by developing rapport with patients, families, medical staff, management, community resources, and other hospital departments to ensure collaboration in the team's patient-centered approach.
3. Works with medical providers to screen, identify, and refers women with substance use disorder to the CARE program with the women's consent.
4. Demonstrates knowledge and initiatives implementing changes to improve clinical service delivery and outcomes, models of care, evidence based practice, and practice development.
5. Works with pediatricians and newborn nursery staff to identify infants with intrauterine drug exposure.
6. Interviews, assesses, and documents the level of substance abuse and/or mental health in pregnant or post-partum patients.
7. Completes all forms and maintains the record of both mom and/or baby through the full care continuum.
8. Identifies and manages any failures in the plan of care and notifies appropriate members of the care team including the physician.
9. Ensures minimal gaps in care during the entire duration of the individualized plan of care, prenatally through up to 2 years postpartum.
10. Develops an individualized plan of care for the patient in conjunction with the multidisciplinary team, reviews with the patient utilizing shared decision making and regularly updates the plan monthly and as needed.
11. Assists with the discharge planning and post discharge follow-up of infants with neonatal abstinence syndrome, including appropriate referrals to community services.
12. Plans, organizes, directs, coordinates, and evaluates the pregnant or post-partum patient's treatment over a period of up to 2 years, in conjunction with the patient's physician, the treatment team, and local support agencies.
13. Provides preventative health education and in-services to patients, staff, agencies, community members and/or providers.
14. In conjunction with the Women's and Children's manager and/or director, develops educational opportunities and resources regarding high risk infants for the newborn nursing staff.
15. Develops, maintains and updates educational guides for expectant mothers with substance use issues.
16. Facilitates access to community services.
17. Provides outreach and early intervention to individuals in the early onset/stage of recovery or remission as well as those experiencing symptoms of relapse during their pregnancy or within 2 years post-partum.
18. Provides in-home visits to evaluate the health of the home environment, development and physical well-being of the infant.
19. Responsible for overall success and compliance of the CARE program requirements.
20. Travels and attends meetings that are essential to the patient's care and for overall accountability to the CARE program.
21. Maintains data entry and analysis of key program metrics to measure success.
22. Maintains working relationships with WV Perinatal Partnership- Drug Free Moms and Babies (DFMB) grant staff and other agencies or institutions in order to provide awareness of recovery as a reality and the availability of recovery coaching.
23. Maintains a positive community and provider image for the CARE program at WVU Medicine Camden Clark Medical Center. Follows all ICARE Behavioral Standards of Excellence.
