We are seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing comprehensive care coordination and management to patients. This role requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.
Required Qualifications:
- Registered Nurse in the state of DE
- Experience completing Assessments, developing Service Plans and Care Plans
- Experience collaborating with PCP’s, Occupational Therapists, Behavioral Health, and Providers
- Experience with ordering DME Equipment ( oxygen equipment, wheelchairs, crutches)
- Experience educating and providing resources for the member’s Social Determinants. They must have experience with discharging members from a Facility setting.• Ability to meet regulatory deadlines.• Experience in geriatric special needs, behavioral health, home health
Job Types: Full-time, Contract
Pay: $40.00 - $43.00 per hour
Expected hours: 40 per week
