Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Care Coordinator (MHN) to join our dedicated team. Since our inception in 1966, we have remained committed to our mission of providing accessible, affordable, and high-quality primary healthcare services to all, regardless of their financial circumstances.

As Care Coordinator (MHN), you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your ability to assist our patients/clients with coordinating their medical and mental needs will further solidify our position as one of the largest providers of community-based primary care in the city.

Key Responsibilities:

• 
  Patient Advocacy: Act as peer support for enrolled patients which includes advocacy as patients navigate the medical system and relationship building with individuals and their families.

• 
  Scheduling: Assists patient with obtaining specialty care visits, Primary Care Provider visits, and ancillary services such as labs/diagnostics, including making appointments as needed.

• Associates Degree or equivalent health related experience
• Proficient in Electronic Health Record, MHN Connect, Microsoft Word and Excel
• Bilingual (English/Spanish) preferred
