Overview:
We are currently in search of a proactive and detail-oriented Care Coordinator. Your primary role will involve staffing and scheduling caregivers to assigned patients and ensuring that all tasks are accurately documented in our software-based system. The ideal candidate should understand the importance of task completion, possess excellent organizational skills, and provide consistent support to our patients and/or caregivers.
Responsibilities:
- Staff caregivers to assigned patients using our software-based system
- Document all notes and tasks in the system accurately and timely
- Verify attendance with both patient and caregiver
- Report and document any incidents / concerns to Management
- Participate in On-Call Hours as scheduled (Bonus Reimbursement available!)
- Maintain accurate schedule data in the software-based system and adjust schedule as needed
- Follow guidelines and compliance procedures implemented by Ideal Home Health
- Proactively resolve, manage, communicate, and address matters related to patients and/or caregivers
Qualifications:
- Strong oral and written communication skills
- Ability to read, interpret, and communicate basic math and data
- Fluency in English, Cantonese, and Mandarin
- Proven ability to multitask, prioritize, and meet deadlines
- Basic knowledge of using a computer
