JOB SUMMARY
Under the general direction of the Director of Behavioral Health, the Care Coordinator provides care coordination to all patients at Heritage, with the focus of assisting patients receiving Behavioral Health services. The Care Coordinator serves as a culturally sensitive liaison to help coordinate and manage the care of the assigned patients. The ideal candidate will have an understanding of the needs, culture, and challenges of the community served by Heritage.
Incumbents in this job may have access to proprietary consumer or company information and must maintain the privacy and confidentiality of this information. All employees must perform their jobs with a focus on risk management, occupational safety and health, and providing internal and external customer service. Employees working in a supervisory capacity must practice effective human resources practices to ensure a healthy working environment.
KEY JOB DUTIES AND RESPONSIBILITIES
  • Provide care coordination, engagement, and retention of patients receiving Behavioral Health services through a trauma informed care lens;
  • Help patients navigate their medical and Behavioral Health services, including scheduling and rescheduling appointment and reminders for Behavioral Health, medical, and dental services;
  • With patient and the multidisciplinary team, develop and implement care planning to manage chronic conditions;
  • Provide chronic care management to eligible patients;
  • Identify barriers to care, adherence and retention in care and help address these barriers;
  • Manage and ensure appropriate follow-up and closure for all provider/member contacts;
  • Manages calls to and from providers regarding both inpatient and outpatient BH authorizations;
  • Make appointments to different specialists for patients;
  • Follow up and track referral results;
  • Help providers obtain medical records on patients;
  • Scan all medical records/reports in patients chat;
  • Obtain daily Prescription Monitoring Program (PMP) forms for providers;
  • Confers regularly with health center staff and other personnel within Heritage Health regarding the health center services;
  • Establish and develop working professional relationships with community groups, and develop targeted population outreach initiatives;
  • Participate in outreach efforts to encourage patients to seek care/treatment within the health center for HIV, prenatal care, chronic illness etc.;
  • Provides patients with education about the benefits of an integrated patient centered approach; raising awareness on health promotion and disease prevention activities;
  • Attends all staff team meetings as assigned;
  • Promotes patient services by resolving their concerns in a timely manner;
  • Experience with Electronic Health Record systems;
  • Excellent written and verbal communication skills are required;
  • Explain, support, and reinforce Heritage’s COVID-19 precautions, including social distancing, telehealth, and vaccines.
SUPERVISORY RESPONSIBILITIES
This position does not supervise others.
REQUIRED KNOWLEDGE, SKILLS, AND COMPETENCIES
  • Associates Degree with three (3) years’ experience in the Health Care field;
  • Experience working with patients with mental illness and substance use disorder, HIV, Hepatitis C, Health Home Agencies and Community Health Agencies.

WORK ENVIRONMENT
The Care Coordinator is assigned to the Health Center. Maybe required to travel to other sites based on the needs of the agency
