TEAM is looking for a Care Coordinatorto provide outreach, screening, assessment, and advocacy to enhance the quality of life for the clients we serve. At TEAM, we provide hard-working individuals with the right care at the right time. We work closely with labor and service industries and provide a holistic approach to understanding issues that occur in their work and home life.
The position is Monday-Friday, Full Time/In-Office and no weekends required. There may be some occasional after-hour call requirements.
The Care Coordinator’s salary is $45,000 -$50,000/year, commensurate with education and experience.

Education:
Bachelor’s degree from accredited four-year college or university with a major in social work, psychology, sociology, or Licensed Practical Nurse, or closely related field. 1 year experience in social services, hospital work, clinic work, utilization review or care coordination in a public or private sector.
Experience:
Minimum 1 year of patient care experience. Behavioral Health, Hospital, Clinic, or school nursing experience is a bonus.
Knowledge, Skills & Abilities:
Must have excellent verbal and written communication skills. Ability to work effectively with clients and in cross-cultural situations. Ability to work well under pressure. Proficient with computer and internet navigation skills including Microsoft Suite and EMR Software.
Job Responsibilities:
- Provide appropriate outreach, screening, assessment, and advocacy for clients.
- Perform necessary follow up with clients.
- Facilitate appointments with best-in-class providers and support patients through ambivalence and motivation concerns.
- Provide short-term care coordination.
- Perform outreach to obtain health screenings, client education, and promote the use of preventative services to improve client health status related to condition management.
- Research rehabilitation, adult day care, home care equipment options.
- Research and locate best-in-class providers, doctors, hospitals, etc. for medical services and for second opinions. Schedule tests, facilitate appointments, and locate in-network providers for clients.
- Coordinate the transfer of medical records, X-rays, and lab results.
- Assist with quality improvement/performance improvement projects including outreach events and activities, data collection and monitoring.
- Promote TEAM services at account functions such as benefits fairs, open enrollment, and other account programs, conferences, and meetings.
- Provide training on-site and remotely as it pertains to TEAM’s services.
- Perform other duties and special projects as assigned.
