FGC's adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.
Essential Duties and Responsibilities:
Schedule: Monday - Friday; based on agency needs
Start Date: ASAP
Work Location: 15400 S. Page Ave., Harvey, IL (position involves travel throughout the community)
Pay Rate: $18.00 - $22.00 per hour

Essential Duties and Responsibilities:
- Accepts, reviews and follows up on referrals from law enforcement agencies received on the 800 line for clients in need of SUD services.
- Conducts "warm hand off" by escorting clients to designated treatment locations.
- Tracks referrals on program spreadsheet and notes time received, date, referral source, and outcome.
- Will coordinate and conduct trainings to personnel from the law enforcement entities in the designated communities on how to facilitate a referral via the 800 number.
- Communicates and assists clients with their transportation needs to treatment sites.
- Responsibilities require active and consistent presence and participation in the community.
- Establish and maintain positive working relationship with law enforcement entities.
- As needed, will provide education on Naloxone to all newly referred clients on the importance of why/how to administer and use it.
- Assists with monitoring patient engagement and communicates with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement.
- Will participate in monthly supervision with Project Coordinator.
- Will participate in staff meetings scheduled by Project Coordinator.
