We’re looking for people who care about people. As an Case Manager for Adult with Disabilities, you’ll be required to:
- Serve as Elim's QIDP for each client's Interdisciplinary Team or Person Centered Plan, coordinate gathering of data for and implementation of each person centered plan or individual service plan within the first 5 days. Each plan would include: Long range strategies, measurable short term objectives, methods of training, collection of data, and means of evaluating progress.
- Develop, implement, and monthly review of each client’s Individual Service Plan
- Serve as a liaison with guardians, residential care providers, and representatives of regulatory agencies
- Provide Leadership and Oversight to Direct Service Staff
- Promote the Use of Assistive Technology
- Develop and Oversee Client Training Activities

Hours:
Monday - Friday 7:30 am to 3:30 pm
Wages & Benefits:
* Competitive starting pay of $22.95/hr – more with experience

Education, Licenses & Certifications:
- Bachelor's Degree in Social Work or Human Service oriented field
- 2 years experience in a social work related field or one year experience working with individuals with cognitive disabilities
- Meet the guidelines of a Qualified Intellectual Disability Professional as specified by state guidelines.
- Knowledge of mental, physical, social impairments, and their probable effects on vocational, personal, social, and
community adjustment
