Assessing patients for both discharge planning as well as psychosocial needs. Working with interdisciplinary team to develop plan of care for transition to the next level. Providing overall care coordination for team of patients that are being covered.

Required skills and minimum years of experience.
Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support. Must have previous experience providing care coordination/discharge planning.
Specific licensures and certifications/registrations required.
Case Management certification preferred, but not required.
Shift or schedule requirements.
8 hour shifts - M-F with rotating weekends. Call daily until 6pm.
Floating to other units/departments.
Could float to multiple adult units throughout the hospital General Medical, Surgical, Cardiac, Neuro, Intermediate, or ICU levels of care.

covering approximately 15-20 patients per day
650 Bed Teaching facility, Level 1 Trauma Center, Stroke Certification
