The incumbent is responsible for providing case management services necessary to ensure that veteran care is coordinated with the VA medical center and community agencies.
Primary duties are to coordinate and provide services supporting the functional abilities of veterans who are striving for independent community living within the HUD-VASH program. The incumbent will work within a Team structure to assist Veterans in meeting their goals. The Social Worker will also work closely with the primary care clinicians and the interdisciplinary teams that assist veteran consumers in meeting their goals and objectives within a psychosocial rehabilitation framework that allow for successful reintegration into community living.

  • Plans, develops and maintains a network of relationships with community resources and other advocates for the homeless to effectively screen and identify Veteran members of the homeless population.
  • Social Worker makes VA or community-based referrals, advocates for scarce resources, confirms referrals are completed and provides intensive follow-up and coordination with these agencies.
  • Develops an appropriate treatment plan. Using known available resources and the initial assessment makes initial and continuing decisions regarding the services available and referrals needed.
  • Documents the patient assessment and all treatment plans in accordance with the requirements of Medical Center policy and requirements of various accrediting bodies.
  • Employs casework methods and techniques to maintain a therapeutic relationship that will help the client work towards the best way of dealing with his/her problems.
  • Provides guidance, emotional support and other assistance. Involves clients in individual or group counseling.
  • Establishes a continuing relationship with the client, evaluating progress towards goals, performing follow-up after referral and adjusts treatment plan when needed.
  • Explains client's treatment and progress towards rehabilitation to third parties with jurisdiction, remaining unbiased in the presentation even though consequences are far-reaching and potentially permanent.
  • Contributes to policy formulation and participates in establishing procedures within the assigned service area.
  • Incumbent may be required to perform case management functions within client's
    home community, and these functions may include transportation of patients.
  • Incumbent may also require training and participation in dealing with violent and disruptive patients.
  • Perform home visits of HUD-VASH apartments to ensure that veterans clinical needs are being addressed. Visits will occur per HUD VASH Directive 1162.05.
  • Incumbent will be a Driver of Veterans.
  • Other duties as assigned.

PREFERRED EXPERIENCE: Prior Case Management experience and experience doing home visits.
Work Schedule: Monday - Friday 8:00a - 4:30pm
Telework: Regular/Recurring 1-2 days per pay period
Travel: 75% of the time

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
BASIC REQUIREMENT GS-9:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Masters Social Work. Accredited by Council on Social Work Education (CSWE). A doctoral degree in social work may not be substituted for the Masters degree in Social Work.

  • Licensure: Social workers at this level are licensed or certified to independently practice social work.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
BASIC REQUIREMENT GS-11
  • Experience: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
  • Licensure: Social workers at this level are licensed or certified to independently practice social work.
  • Education: In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
