The Behavioral Health BSW Care Coordinator is responsible for case management, care coordination, and crisis intervention with children, adolescents and their families in an acute outpatient setting. Primary responsibilities will include management of the clinic’s case work queue, supporting clinical interventions, coordinating, and co-facilitate group therapy interventions. The coordinator is expected to offer services within expected practice standards and as informed by the professional literature.

• Manages care coordination of a high acuity patient population, utilizing work queue to support case management for team’s patients by placing referrals and ensuring follow up contact. Supports care coordination activities including scheduling, data entry, report generation, distribution, and communication with internal and outside entities.
• Maintains knowledge of internal and external resources, as well as wait times and referral criteria. Reviews incoming referrals in consultation with clinical manager and clinical lead supervisor. Strives to maintain positive patient satisfaction.
• Assists with direct patient care in acute situations, ensuring supervision and support clinical interventions. Completes clinical charting and other related administrative paperwork in accordance with policy standards and program operating procedures.
• Coordinates and supports therapy groups. Co-facilitates group therapy, with supervision.
• Screens for lethality, completes risk assessment, and employs clinically appropriate interventions for crisis, such as de-escalation, psycho-education, and safety planning, with supervision.
• Utilizes EPIC for documentation of care coordination activities in accordance with policy standards and collaborates consistently with the inter-disciplinary team. Assists with data gathering and information tracking as necessary for process improvement.
• Attends in-services trainings, peer review and other interagency and supervisory activities. Meets regularly with identified supervisor. Attends routine staff meetings & client treatment rounds.
Education: Bachelors degree from an accredited university or professional school of Counseling or Social Work.
Licensure: Appropriate Ohio licensure; must obtain Licensure in Social Work through the Ohio Counselor, Social Worker, Marriage and Family Therapy Board.
Valid Ohio driver’s license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
